The aging situation of China’s population is becoming more and more serious.The increasingly large group of the elderly puts forward higher requirements for elderly care services,old-age security and medical and health services.Among them,the demand for the prevention and treatment of chronic diseases is particularly prominent.At present,the management of chronic diseases in China is still in the exploratory stage.The management and treatment of chronic diseases still mainly depend on patients,their families and their communities.Whether patients can correctly understand their own chronic diseases,follow medical advice,form a good lifestyle,and have channels to obtain scientific knowledge and medical and health services related to their own health,It is the most basic link in the management and treatment of chronic diseases.Health literacy has become an important factor affecting the self-management and quality of life of patients with chronic diseases,which is of great significance to the prevention and treatment of chronic diseases of elderly patients.Social workers should carry out more exploration and Research on improving the level of health literacy of elderly patients with chronic diseases from their own professional perspective.Based on the data collection of the service object,this study fully understands the physiological,psychological and social characteristics of the service object,defines the needs and expectations of the service object,formulates a scientific,reasonable and targeted case service plan from the perspective of social support network,and uses the professional skills and methods of social work to intervene the service object,Through six stages of case service activities,from the three dimensions of knowledge,belief and behavioral health literacy,interview and serve the service object and its interpersonal response network,and guide the service object to adjust the network resources with destructive,stressful and other negative support into positive support,improve the ability of service objects to use resources and the support ability of social support network,and form a common understanding of chronic disease health literacy in the network,so as to improve the level of health literacy of service objects.By comparing the level of interpersonal relational and healthy literacy level of the service objects before and after the intervention,it is found that it is feasible to improve the health literacy of elderly patients with chronic diseases from the perspective of social support network,and it also improves the interpersonal relationship of the service objects.
