With the rapid ageing of our population and the deterioration of their physical functions,the elderly are the main group suffering from chronic diseases.This phenomenon and its associated problems have led to the creation of advance care planning,which aims to help older people with chronic illnesses to make their own medical choices to improve their quality of life at the end of their illnesses,while avoiding unnecessary medical treatment and improving their physical and mental well-being.However,most elderly patients with chronic illnesses and their families are not prepared to make advance care planning due to various influencing factors,and their knowledge is limited.Social work is based on scientific knowledge,with the aim of helping people to help themselves,and is always guided by altruistic values,with the aim of contributing to the solution of this problem.This study is guided by empowerment theory and cognitive behavioural theory,and uses a combination of documentary analysis,interviews,questionnaires and observation methods to investigate the feasibility of social work case services in improving the readiness of elderly patients with chronic illnesses for advance care planning.In the first stage,we conducted communication and observation with a number of elderly chronic disease patients living in the urban community,combined with the results of the questionnaire survey,and selected two elderly chronic disease patients with low readiness for advance healthcare planning due to different influencing factors to carry out targeted case interventions.This confirmed the effectiveness of social work case services in improving the low readiness of elderly patients with chronic illnesses for advance healthcare planning.The study concludes with a summary and reflection of all the case studies,and makes recommendations on the shortcomings of the study,with the aim of providing a reference for subsequent practice. |
