Case Work Research On Health Management Of Elderly Patients With Chronic Diseases From The Perspective Of Ecosystem Theory

Today,with the rapid development of ageing,the health of the elderly population has become a common concern in society.The health status of the elderly directly affects the health level of our people,and the health management of the elderly with chronic diseases cannot be ignored.Compared with other types of diseases,chronic diseases,with its characteristics of complex aetiology,long onset cycle and low cure rate,not only have a long-term negative impact on the physical health of patients,but also have a serious and far-reaching impact on their mental health.The chronic disease has a long course and will not be immediately fatal.It requires long-term self-management of patients.As long as it is properly managed,they can also enjoy a high-quality life.However,according to the current survey,most elderly people lack chronic disease health management awareness and effective chronic disease management methods,and there are not many elderly people who can carry out scientific health management by themselves.Based on the impact of chronic diseases on the health of the elderly,the elderly have an urgent need for disease health management.This study takes F community Y elderly patients with chronic diseases as the service object,analyses the problems existing in the service object based on ecosystem theory,conducts research on the cognitive function evaluation,daily life self-care ability,home environment assessment,etc.,to clarify the dilemma faced by the case owner,formulate a scientific service plan,focussing on promoting the case.The growth and development of the main individual,improve and change the ability of the case owner to adapt to the environment,and remove the unfavourable factors of the case owner in the environment,so that the environment can better meet human needs,so as to achieve better interaction between people and the environment.Specifically,we provide services for the service object from the aspects of home environment improvement,scientific movement,emotional guidance,etc.,so as to correct the unreasonable lifestyle of the service object and improve the dangerous home environment.Improve the cognition of diseases of service objects,correct the misconception of elderly patients with chronic diseases,and treat chronic diseases correctly;learn the health knowledge of chronic diseases,master the specific self-care skills of chronic diseases such as hypertension and diabetes;formulate a health management plan for chronic diseases suitable for themselves,explore scientific exercise methods,and develop well.Healthy behaviour habits;be able to master emotional management skills,reasonably vent bad emotions,and relieve anxiety caused by diseases;improve the home environment and create a safe and comfortable living environment.Improve the health management awareness of the case owner,promote the knowledge of chronic diseases,form a healthy lifestyle,and further improve the health level and quality of life of the elderly with chronic diseases.Through evaluation and follow-up visits,based on the theoretical perspective of the ecosystem,the intervention of case work method can be used to personalise the problem of the case owner,effectively improve the service object’s cognition of the disease,master the specific self-care skills of chronic diseases such as hypertension and diabetes,and explore scientific exercise methods to develop good health.For habits,reasonably vent bad emotions,improve the home environment,and create a safe and comfortable living environment.Improve and change the ability of the case owner to adapt to the environment,and overcome the unfavourable factors in the environment,so as to make the environment better meet human needs and better promote the improvement of the quality of life of the case owner.
