Patients with chronic diseases are different from patients with other diseases.At the physiological level,chronic diseases are often difficult to eradicate and accompany them for life.Often one person is more ill.Some patients will even have a strong sense of pain or even unable to act normally.If they do not have effective self-management,they will even endanger life and health.It greatly affects the normal life of patients.At the psychological level,chronic diseases can cause strong psychological fear when the patient is seriously troubled and uncertain for a long time.Take cirrhosis as an example,most people believe that it is a precursor to liver cancer,leading most patients to have a strong reaction when they learn that they have cirrhosis,believing that their limit is approaching,which in turn will lead to many behavioral and spiritual changes,often resulting in despair,fear,anxiety,irritability,pessimism and despair,etc.Bad mood.Due to the curing of living habits for many years,the possibility of sudden changes in the living habits of chronic patients for many years is lower,and the support and help of hospitals under the novel coronavirus epidemic is also reduced.At this time,patients urgently need the care and warmth of professionals,and need the consideration,care,support and help of relatives and friends.This study takes elderly Z patients with cirrhosis as an example,analyzes and sorts the difficulties encountered by the microsystem,meso system and macro system from the perspective of ecosystem theory according to the interview content of social workers with Z and their family,and based on the theory of social support network.Carry out orderly intervention and give patients enough humanistic care throughout the process.The results of the intervention are assessed by achieving a target scale and a satisfaction survey of the owner’s family.Enable Z to unload its inner burden,resume normal communication with members of the family,and receive full support from available resources.The research in this case found that case management can have a positive effect on comprehensive help for chronically ill patients,effectively relieve emotional stability and psychological changes in patients,and reduce family care pressure.Improve the patient’s lack of self-awareness,make him have a new understanding of the ultimate problem,and improve the centripetal force between the patient and his relatives.It has been proved that case management methods are effective in intervention services for chronic patients.At the same time,it also shows that medical treatment is indispensable for various chronic patients,but in some cases humanistic care is more important. |
