At present,the aging situation of China’s population is increasingly serious,the trend of hollowing out in rural areas is deepening,the Disease burden caused by chronic diseases of the elderly is increasing,and the related problems of chronic diseases of the elderly in rural areas are becoming increasingly prominent.The understanding and management of chronic diseases in elderly patients directly determine their living standards.Compared to cities,rural areas have underdeveloped economic development and relatively scarce medical resources.How to effectively prevent and control chronic diseases in rural areas has become a topic of increasing concern.With the development of social work in social services,social workers have become indispensable roles in various fields.However,there is still little research on the intervention of social work in the health management of chronic diseases in rural elderly.Based on this,this study selected Z Village,Dali City City,Yunnan Province,to carry out field research,and selected the elderly patients with chronic diseases in Z Village as the research object,mainly using semi-structured interviews,participatory observation,and Ecological systems theory and cognitive behavior theory for analysis.The research mainly focuses on the health management of elderly chronic disease patients in Z Village by multiple subjects and the self-management of elderly chronic disease patients in Z Village.Among them,multiple subjects mainly elaborate from two aspects:government agencies and health departments.Self management mainly elaborates on disease cognition,medical treatment and medication,and prevention and control of chronic diseases,in order to understand the current cognitive status and health management situation of elderly patients in Z Village towards chronic diseases.The results show that the existing elderly patients in Z village generally have biases in their understanding of chronic diseases and outdated medical concepts;The access to health education is single and the treatment level is backward;Problems such as inadequate medical and health systems in rural areas.Based on the existing problems,the reasons are analyzed from three levels:micro,meso,and macro.At the micro level,the author analyzes from three aspects:personal factors,individual medication compliance,and family level;At the meso level,analyze from the perspectives of peer groups,clans,and organizations;At the macro level,the problem is attributed to two aspects:cultural beliefs and the medical system.Based on the above analysis,this study conducted a case intervention design from the perspective of social work,using cognitive behavioral theory.Through four stages,a total of eleven services were provided to correct the biased cognition of service recipients about chronic diseases,master knowledge and methods of health management,and enhance their health management abilities.Finally,further demonstrate the feasibility and necessity of social work intervention in the health management of elderly chronic disease patients in rural areas from both theoretical and practical perspectives,and reflect on the shortcomings of this study.
