Study On The Intervention Of Social Work In The Health Management Of The Elderly Patients With Chronic Diseases

With the aging of the population and the increasing number of chronic non-communicable diseases in the elderly,how to effectively prevent and control chronic diseases has been paid more and more attention by the public.The cognition and attitude of the elderly patients with chronic diseases directly determine the quality of their disease control.Based on this starting point,this study selected the elderly chronic patients in the Pearl Community of G City as the research object.In order to analyze the problems existed in the health management of the elderly patients with chronic diseases,the present situation of self-health management of the elderly patients with chronic diseases in this community was analyzed.The results showed that the health management of the elderly patients with chronic diseases in this community had the following main problems: poor self-restraint,weak awareness of health management.single management mode of chronic diseases and difficult to change the concept of treatment.the social support network is not perfect,lack of positive guidance of social support.Based on the above problems,this study discussed the necessity and feasibility of social work intervention in the health management of elderly patients with chronic diseases from three aspects.The intervention of social work in the health management of elderly patients with chronic diseases was in line with the theory of empowerment,which aimed to achieve self-control and self-improvement at the individual level.At the same time,it also confirmed the important role of informal social support network for the elderly,especially for the elderly with chronic diseases.Second,it responded to the needs of the elderly patients with chronic diseases and studied the needs of the elderly patients with chronic diseases on the objective understanding of chronic diseases.The demand of raising self-health management consciousness was positive to the demand of informal social support.Thirdly,it made up for the deficiency of health management for chronic patients in the community.As a result,we were involved in the health management of elderly people with chronic diseases throughself-directed behavioural change groups and social support groups,supplemented by the necessary casework.We used treatment mode group work to strengthen the elderly patients with chronic disease awareness of chronic disease,improved the elderly chronic disease patientsundefined awareness and ability of health management,and used development model group work to enable the elderly with chronic diseases to obtain more positive and positive informal social support to enhance their decision-making ability and ability to be responsible for their own lives.To explore the practical guiding significance of task-centered model and crisis intervention model in the health management of elderly patients with chronic diseases,and to evaluate the effectiveness of practical work.The necessity and feasibility of social work intervention in the health management of elderly patients with chronic diseases are further demonstrated in theory and practice.
