Project Research On Improving The Health Management Ability Of Elderly Patients With Chronic Diseases

As the elderly grow older,their physical functions gradually decline,and different types or degrees of chronic diseases begin to appear.They not only have to endure physical pain,but also bear enormous psychological pressure.With people’s attention to health problems,how to improve the health status of elderly patients with chronic diseases and improve their health management ability has gradually become the focus of general attention of all sectors of society.The health management project in social work is an important breakthrough in the field of social work for the elderly,which plays an important role in improving the health management ability of elderly patients with chronic diseases.Based on this,this study relies on the project of "Chronic Elderly Support Program",under the guidance of Maslow’s hierarchy of needs theory,social support theory,self-efficacy theory and other social work professional theories,and uses research methods such as participatory observation,interview and literature review to evaluate the problems and needs of elderly patients with chronic diseases,and through the cognitive improvement program,emotional management program and social support program for elderly patients with chronic diseases.According to the implementation effect of the project,it is found that the improvement of health management ability has a positive effect on the healthy life of elderly patients with chronic diseases,the resource link has an important significance in improving the health management ability of elderly patients with chronic diseases,and the improvement of health management ability has a positive role in promoting the home safety of elderly patients with chronic diseases.The research shows that there are some problems in the project,such as the service effect is difficult to present in a short time,the service beneficiaries are limited,the funds are limited,and the ability of social workers to serve the elderly is limited.In view of the above problems,some suggestions are put forward,such as extending the service cycle of the project,broadening the coverage of clients,expanding the project capital chain,and improving the ability of social workers to serve the elderly.
