Objective: The purpose of this study is to establish the medical staff job satisfaction scale,test reliability and validity of the scale.Through the scale to understand the situation of job satisfaction of medical staff in a third-grade hospital in Shenyang.The related influencing factors were discussed from the aspects of demographic characteristics,working conditions and doctor-patient relationship.To provide scientific suggestions for improving the job satisfaction of medical staff,so as to better promote the improvement of medical service quality.Methods: Through a large number of literature review,combined with the actual medical status in China,referring to the mature measurement scale,we developed a medical staff job satisfaction scale,and invited medical experts to evaluate it.In this study,a questionnaire survey was conducted among medical staff of Shengjing Hospital from 2018 to 2019 with random sampling method.A total of 459 patients(effective response rate 91.8%)were effective responses,which were the subjects of this study.SPSS 22.0 and SAS 9.2 software were used for data statistical analysis.Cronbach’sα coefficient was used for reliability test.Pearson correlation method was used to test the aggregate validity and discriminative validity of the scale.Exploratory factor analysis method was used to test the structural validity of the scale.Man-Whitney U test and Knuskal-Wallis test were used for univariate analysis.The consistency and collinearity among the variables were determined by Kappa value and Pearson analysis.Multivariate analysis was carried out using generalized linear regression(GLM)in the general linear model.Result: No item of rationality less than 1 was found in the job satisfaction scale of medical staff.Internal consistency test showed that Cronbach’s α coefficient of all dimensions of the scale was above 0.8.Each item has the highest correlation with the score of its dimension.The results of exploratory factor analysis showed that the scale of job satisfaction of medical staff could put forward four main factors,including basic condition,self-worth,working atmosphere and service mode.The average scores of four dimensions were: basic condition(3.94±0.69),self-worth(4.18±0.63),working atmosphere(4.32±0.61)and service mode(4.32+0.57).Univariate results showed that working time,work stress,complained experience and doctor-patient relationship were correlated with basic condition satisfaction(P<0.05);Working time,work stress,complained experience,doctor-patient relationship and patient distrust were correlated with self-worth satisfaction(P<0.05);Professional title,working time,complained experience,doctor-patient relationship and patient misunderstanding were correlated with working atmosphere satisfaction(P<0.05);Working time,work stress,complained experience,doctor-patient relationship,patient high expectation,patient distrust and patient misunderstanding were correlated with service mode satisfaction(P<0.05).The results of multivariate analysis showed that the most significant factors affecting the satisfaction with basic condition were work stress(β=-0.268,P<0.001),working time(β=-0.225,P<0.001)and doctor-patient relationship(β=-0.221,P=0.003).There are doctor-patient relationship(β=-0.312,P<0.001)、working time(β=-0.198,P<0.001)、work stress(β=-0.181,P<0.001)、patient distrust(β=-0.165,P<0.001)、complained experience(β=-0.106,P=0.021)and patient misunderstanding(β=-0.104,P=0.029)of self-worth satisfaction.There are doctor-patient relationship(β=-0.310,P<0.001)and working time(β=-0.226,P<0.001)of working atmosphere satisfaction.There are working time(β=-0.220,P<0.001),patient distrust(β=-0.182,P<0.001),working pressure(β=-0.181,P<0.001),patient misunderstanding(β=-0.146,P=0.003)and patient high expectation(β=-0.101,P=0.028)of service mode satisfaction.Conclusion: The job satisfaction scale designed in this study has good rationality and good reliability and validity.In this study,the scores of all dimensions of job satisfaction of medical staff were around "relatively satisfied",which was at a high level.The score of satisfaction of basic condition was relatively low.Work stress and working time were the primary factors affecting the satisfaction of basic condition and service mode satisfaction.The factors related to doctor-patient relationship have a significant impact on the job satisfaction of medical staff,especially is the primary influencing factor of self-worth and working atmosphere satisfaction.Hospitals should fully understand the needs of medical staff,optimize and perfect their own management system and promotion system.It is important to play the regulating role of the hospital,reasonably allocate the work tasks of medical staff,coordinate the work collocation,reduce the pressure faced by medical staff in many aspects,so as to reduce the pressure of medical staff.Faced with the current tense doctor-patient relationship,hospitals should effectively deal with medical disputes,strengthen safety management,and ensure the safety of medical practice.At the same time,the society should improve the medical knowledge of the public,and the media should also play an active role in publicity,reduce patients’ misunderstanding and distrust,and increase the job satisfaction of medical staff,so as to improve the quality of medical service.
