Reliability And Validity Of The Chinese-version Physician Trust In The Patient Scale And Preliminary Application

Objectives To translate and compile the Chinese version of the Physician Trust in the Patient Scale and examine its reliability and validity;To measure the doctor-patient trust and analyze its preliminary influence factors.Methods Obtained consent and authorization of the original scale's author,the Physician Trust in the Patient Scale(PTPS)was translated and re-translated in strict accordance with the Brislin's translation principle,and revised according to Chinese culture to form the Chinese version of the Physician Trust in the Patient Scale.It was modified based on the result of pilot study in 20 emergency doctors.The content validity was evaluated by 6relevant professional experts.Phase 1: To evaluate the reliability and validity of the scale,300 doctors of public hospital in Tangshan were investigated with the Chinese version of the Physician Trust in the Patient Scale and demography questionnaire by random cluster sampling method.Phase 2: To study the current situation of doctor-patient trust of public hospital in Tangshan and its influence factors,451 doctors of 1 Tertiary hospital and 3 secondary hospitals and 5 community hospitals in Tangshan were investigated with the Chinese version of the Physician Trust in the Patient Scale and demography questionnaire and influence factors questionnaire by stratified random sampling.Set up database by Excel and analyze the data by statistical analysis software SPSS 23.0 and AMOS 13.0 after questionnaire.The content validity was examined with content validity index(CVI).The structure validity was examined with exploratory factor analysis and confirmatory factor analysis.The Cronbach ? coefficient and Pearson correlation analysis were used to examine the internal consistency reliability and test-retest reliability.Descriptive statistics,independent samples t-test,One-Way ANOVA and multiple linear stepwise regression analysis were used to analyze the current situation of doctor-patient trust of public hospital in Tangshan and its influence factors.Results 1 The Chinese version of the Physician Trust in the Patient Scale.The scale included 12 items and 2 dimensions: patient role and respect relationship.2 The reliability and validity of the Chinese version of the Physician Trust in the Patient Scale.1)The validity of the scale.Kaiser-Meyer-Olkin measure was 0.90 and Bartlett's test of sphericity reached significant level(?2=1044.75,df=66,P<0.01),which was suitable for factor analysis.Two factors were extracted by exploratory factor analysis,which could explain67.391% of the total variance.In the confirmatory factor analysis,?2/df was 2.595,RMSEA was 0.072,NFI was 0.873,RFI was 0.924,IFI was 0.918,CFI was 0.917,PNFI was 0.701 and PGFI was 0.581.The item content validity index ranged from 0.83-1.00.S-CVI/Ave and S-CVI/UA were 0.96 and 0.75.2)The reliability of the scale.The Cronbach ? coefficients were 0.921 for the total scale,and 0.903 and 0.886 for its 2subscales(all P<0.01).The Cronbach ? coefficients if item deleted ranged from 0.910 to0.919 with small changes.The test-tested Pearson correlation coefficients of the total scale and its 2 subscales were 0.946,0.933 and 0.945(all P<0.01).3 The doctor-patient trust of public hospital in Tangshan and its influence factors.1)The doctor-patient trust.The mean score of the scale was 35.85±6.604,ranged from 12 to 60.2)The single-factor analysis of the doctor-patient trust of public hospital in Tangshan.The doctor-patient trust score was significantly different from 28 factors such as gender groups,age groups,working time groups,monthly income groups,type of hospital,chronic diseases or not,have experience in hospital or not,number of surgery,accompany experience or not,relationship with coworkers,the nurse's respect,work time per week,the satisfaction of working environment and treatment,etc(P<0.05).3)The multiple-factor analysis of the doctor-patient trust of public hospital in Tangshan.In the multiple linear stepwise regression analysis,monthly income(X2),type of hospital(X5),the nurse's respect(X11),the satisfaction of working environment and treatment(X13),relatives/colleagues experienced doctor-patient dispute or not(X19),encountered unreasonable troublesome or demanding requirements of patients/families(X20),the importance of doctor-patient communication(X25),laws and regulations cannot safeguard their own rights and interests(X27),practice affected by the medical care system(X28)were included in the regression equation in which the doctor-patient trust score was dependent variable: y=22.706 + 1.142(X2)+ 2.033(X5)+ 1.396(X11)+ 0.803(X13)+ 1.873(X19)-1.134(X20)+ 1.907(X25)-1.365(X27)-0.550(X28).F was 25.048,P was0.000,R2 was 0.353.Conclusions 1 The Chinese version of the Physician Trust in the Patient Scale had good validity and reliability,and could be used to measure doctor-patient trust in China.2The doctor-patient trust of public hospital in Tangshan was at medium level and did not reach ideal level.3 Monthly income,type of hospital,the nurse's respect,the satisfaction of working environment and treatment,relatives/colleagues experienced doctor-patient dispute or not,encountered unreasonable troublesome or demanding requirements of patients/families,the importance of doctor-patient communication,laws and regulations cannot safeguard their own rights and interests,practice affected by the medical care system were main influence factors of doctor-patient trust.
