At present,hypertension patients are at a low level in self-management cognition and behavior,and they can not have a perfect social support network.At the cognitive level,patients with hypertension lack the learning of hypertension knowledge and ignore the importance of self-management for hypertension;at the behavioral level,the ability of self-management needs to be further improved;at the social support level,there is a lack of support network for self-management and the opportunity to establish support network.Based on the theory of social learning,this paper applies the social learning group work mode to the self-management of hypertension patients,and carries out six group activities for hypertension patients.At the end of the group,the whole group was evaluated by the group effect evaluation and process evaluation.The results showed that after the end of the group,hypertensive patients learned more hypertension knowledge,enhanced their awareness of self-management,changed their behavior in daily life,and improved their ability of self-management.At the same time,hypertensive patients A peer system of mutual support has been established,the connection with the community has been strengthened,and an effective social support network has been formed.Therefore,the conclusion of the study is: under the guidance of social learning theory,using the method of group work,it is effective to intervene the research of hypertension patients' self-management through social learning group mode.At last,the paper reflects on the deficiencies in the research and puts forward countermeasures and suggestions: appropriately extending the time of group existence;improving the role of family support in self-management and improving the knowledgestructure and practical ability of social workers.
