Practical Research On Improving Self-management Ability Of Elderly Patients With Chronic Diseases

With the growth of age,the physical health of the elderly began to decline,and different types or degrees of diseases began to appear,especially the common chronic diseases of the elderly.As a chronic disease of cardiovascular and cerebrovascular diseases,hypertension has a high incidence in the elderly population,and its mortality rate is high and rising.In addition,our country medical treatment resources are in short supply,the community management service work and hypertension prevention and control work is still at low level.Again due to the particularity of high blood pressure,chronic disease,the patient's preventive intervention and daily health care usually need to implement in the home for a long time,patients and patients families become chronic disease self-management and implementation,but the patient and family caregivers and inadequate value degree and ability of self management lead to high blood pressure patients and their family situation is not optimistic.Therefore,it is imperative for social work to intervene in the self-management of chronic hypertension in the elderly at the community level.Through sorting out relevant literature,it is found that there are many studies on the involvement of social work in the self-management of hypertension in the elderly in foreign countries,but the current domestic studies on chronic hypertension are concentrated in the medical field,and few studies on the involvement of group work are available for exploration and research.This study through the questionnaire method,interview method,literature research and action research method and other research methods,And this paper analyzes the self-management status and needs of elderly hypertension patients in H community,nanhai district,foshan.adhering to the concept of social work,the article uses the method of group work will work for high blood pressure,chronic diseases research object service.The daily life management,medical management of diseases and emotional cognitive management of thesubjects were scientifically guided and the overall service effect was evaluated,so as to provide a practical solution and reference for better meeting the self-management needs of elderly patients with hypertension.In this study,it was found that the group work has a significant advantage in the intervention of self-management education of hypertension in the elderly.Establish a mutual support system to help others help themselves;Integration of community resources,multi-linkage intervention;To significantly improve participants' self-confidence and hypertension self-management ability.The development of the elderly chronic disease patients' self-management ability improvement group is a process of practical exploration,but there are still some deficiencies,which need to be further explored and studied.
