Group Work Involves Research On The Health Management Of Elderly Hypertensive Patients

At present,China's aging is serious,the problem of chronic diseases in the elderly is prominent,and hypertension is the most common chronic disease synod for the elderly,and the incidence of hypertension in the elderly is high,almost one of the two elderly people have high blood pressure.High incidence of hypertension,long course and incurable,these characteristics determine that the treatment process of hypertension is bound to be a "long-lasting war",and through effective and scientific health management of patients can control blood pressure,improve the condition.As a place of social old-age care,the prevalence of elderly people in the cluster and hypertension is higher,which has an important impact on the healthy life of the elderly.If people with hypertension in old-age institutions can improve their own health management ability,it can help them to control the disease more favorably,promote health-promoting behavior,help improve the quality of life of hypertension patients,and better live in old age.With the continuous development of social work,social work plays an increasingly important role in social services,but there are few researches on the health management of elderly people with high blood pressure.The thesis selected S old people with hypertension as the research object,using literature research method,questionnaire survey method,interview method and participation observation method to carry out research,using Maslow's demand level theory,group dynamics theory,social support theory and energy enhancement theory to carry out demand analysis,adhering to the concept of social work,using the method of group work for the elderly institutions to improve the health management capacity of hypertension patients.First of all,the author from the S old-age institutions high blood pressure elderly health management needs,with the elderly medical staff to assess their needs,based on the health management needs of the elderly with high blood pressure,problems and the physical condition of the elderly,psychological quality,etc.To determine the group objectives,the nature of the group and the group plan.This study developed and implemented six group activities,from health knowledge,exercise,peer learning and other aspects to meet the health management needs of the elderly with high blood pressure and improve their health management ability,and according to the performance and feedback of the team members in a timely manner to adjust the service program.Finally,the effectiveness evaluation of practical intervention,found that the team members of the high blood pressure health knowledge to improve,exercise behavior to maintain good,peer experience learning to improve,the results further prove that the team work in promoting the elderly hypertension health management capacity in the elderly has a certain role and effect,social work intervention is necessary and feasible.At the end of the study,the author summarizes the practical process in two points the intervention of the team's work is conducive to the satisfaction of the health management needs of the elderly with hypertension,the improvement of the social support network has certain importance,and the ethical dilemma in the service and the continuity of the service effect is difficult to reflect on.This study provides some reference experience for social workers in the service of chronic disease groups,especially the elderly hypertension group,and further clarifies the professional orientation of social workers in social services and old-age services.
