As a chronic disease,diabetes affects the daily life of the elderly.If there are serious complications,it will threaten the life of the elderly.Therefore,it is very important to strengthen the education of diabetes self-management for the elderly diabetic patients,so as to promote the elderly diabetic patients to do a good job in self-management,so as to reduce the negative impact of diabetes.This paper takes the elderly diabetic patients in G community of Kunming City as the research object,collects the research data by interview,observation and questionnaire,carries out group activities of self-management education for the elderly diabetic patients,and studies the self-management education of the elderly diabetic patients by using group work method.Based on the needs assessment,social workers recruited 5 elderly diabetic patients to form an education group,designed and carried out 9 group activities.In the process of the activity,on the one hand,social workers integrate community medical and health resources,invite community retired doctors as teaching experts to the activity site to teach professional knowledge of diabetes self-management,provide technical guidance,so that team members can fully and systematically understand the basic knowledge of diabetes,learn skills related to diabetes self-management,including skills in food management,sports skills Complications prevention skills,blood glucose monitoring skills and medication management skills.On the other hand,social workers give full play to their professional advantages to teach the elderly patients with diabetes emotional management skills.At the same time,use group power to promote members to share experience and strengthen mutual support among members.The results of service evaluation showed that the effect of group work on self-management education in elderly diabetic patients was significant.In addition,after reflecting on the influencing factors of service effectiveness,professional knowledge,group work and self-management education of elderly diabetes patients,from the perspective of social work,it is necessary to establish a long-term mechanism of community elderly healtheducation,build a support network of family peer combination,and improve the elderly’s social impact The awareness of work and the ability of social workers to cooperate in interdisciplinary fields should be improved.
