Research On Modeling And Implementation Of Chronic Disease Management Pathway

More than 30%of Chinese people have chronic disease and the number is increasing rapidly.Chronic disease has become the leading cause of premature death in China.How to improve the quality of chronic disease management has become an imminent issue of chronic disease related research.Different chronic disease management methods such as method based on guidelines,management model,clinical pathway can not provide canonical and executable instruction for practical chronic disease management tasks and procedures.Based on this situation,research team designed and applied chronic disease management pathway which had been proved to be clear and enforceable for a standardized process of chronic disease management.However the chronic disease management pathway now was described and applied as flow chart and executable forms which severely limited its expression range and scalability.This paper aims to research on a canonical modeling method of chronic disease management pathway and to make it more executable and more scalable.This paper includes:(1)Research on the unified task set for chronic disease management.Based on 27 chronic disease management guidelines,this paper summarized the management tasks for common chronic disease and found that these tasks share high degree of similarity.These tasks can be summarized as an unified task set for chronic disease management which includes diagnose,risk level assessment,Hierarchical management etc.(2)Build ontology for chronic disease management.Follow rigorous scientific process to build the ontology for chronic disease management based on the domain knowledge reconstruction of chronic disease management and those existing ontologies.Chronic disease management ontology became the knowledge foundation of pathway modeling.(3)Design and implement the inference engine for chronic disease management.Summarize the management procedure of each task for hypertension management and model it with SWRL rules.Design the inference service engine for hypertension management and implement it in real management environment and evaluate its application results.This paper focused on the modeling method of chronic disease management pathway and to make it more executable and more scalable based on ontology and SWRL rules.This method has proved to be effective in improving patients' control rate of chronic disease and their compliance to the management.Also this method can improve the working efficiency of healthcare workers.
