Chronic Disease Management system, was launched by Xi'an Jiao Tong University Affiliated Hospital answering "Techcology Huimin Plan" of Shaanxi Province. The Hypertension part was put on line in 2013. Heart failure part was put on line in 2014. Coronary heart part will be put on line in the e nd of this year. C DM system provides services to 9 major roles, such as doctors, patients, researchers, and so on. The functions of CDM includes creating hypertension or heart failure archives, creating hypertension, heart failure follow- up management archives and querying hypertension classification or analying medical data, such as patient information export and so on.Since the first half of 2013 the blood pressure part of the line, there ha been a total of 5000 patients with the user, potential users of 15000 archives in CDM. The heart failure management system was developed in 2014 and it was integrated into CDM. Coronary heart disease will also be added in the future. As CDM users are growing gradually, there will be a greater pressure on the system in the future. This paper mainly research on how to provides HA&HP services to doctors and patients, and other types of users can get a quicker reply from CDM.CDM is B/S structure, which uses Struts2, Spring and Hibernate as the framework of project development. The Servlet container is Apache Tomcat, which is used in the process of exploring the high performance C hronic disease service. This paper mainly does the following work:1. Get a well known of the two part of CDM, including hypertension and heart failure part.2. Studying technical structure of the Web system, including the Nginx, Tomcat, My SQL, including the configurations and physical deployment. This paper also introduce the research on the mechanism and adjustment strategy of Java Web system in dealing with high concurrency.3. Adjusting the system structure from the original single Tomcat container which handling all the requests of the structure into a multi- level load balancing architecture. The new structure includes the Nginx, Galera C luster. It can expand the design in persistence layer, business layer, appearance layer of CDM.4. Some modules cost too much time. We can optimized it. For example, the directory tree of the main panel which was made by Ext JS could be replaced by a lightweight front-end framework, Easy UI.5. The paper also do some tests to achieve load testing, the existing system for testing and comparison analysis, verify the effectiveness of the improvement and deficiencies. As far as possible to find no discovery of potential performance bottlenecks, system may lead to downtime of key points, and modify the corresponding module of the program code and configuration files, so that the system becomes more rapid, robust, and can be used.6. In the end,this paper makes a further outlook for the CDM. It also introduces the concept of distributed of business layer and the persistence layer. It can send the message queue so that the business can be divided into finer granularity module. So each module will became an independent node. Make sure that the program will not become a bottleneck in the system.
