Design And Application Of Management Pathway For Hypertension

Chronic disease is a long-term disease and the incidence of morbidity and mortality has increased rapidly year by year.Improving the treatment rate and control rate of chronic disease is imminent.Chronic disease management is the main means of preventing chronic disease and provides comprehensive,continuous and proactive management for patients with chronic disease through specialized health care workers.The main way of managing patients with chronic disease outside the hospital is to offer face-to-face or remote intervention for patients under the guide of chronic disease management guidelines.Chronic disease management model represented by Chronic Care Model has been used widely at abroad.However,the chronic disease management guided by guidelines or models has not defined the implementation process of chronic disease management resulting in uneven promotion of chronic disease management in different medical institutions and the management efficiency is low.Clearly,chronic disease management requires a clear and enforceable management pathway with accurate time and standardized process to guide.In order to improve the service quality of chronic disease management,this paper designed and applied the management pathway of chronic disease.This paper summarized ideas of chronic management pathway,and chose hypertension as a research disease case,aiming at the design of management pathway firstly.Then based on management pathway of hypertension,the working platform for doctors was designed and developed to help doctors carry out the management pathway.Finally,this paper carried out clinical application evaluation based on the management pathway of hypertension.Detail works are as follows:? Summarizing the ideas of management pathway of chronic disease.Finished the design of management pathway aiming at hypertension based on evidence-based medicine and defined the management process of managing patients for doctors clearly.? Developing the management platform of hypertension management under the instruction of hypertension management pathway to assist doctors to carry out the pathway of hypertension management better.The platform had the functions of health data visualization,patients grading evaluation display,follow-up information record and so on.? Evaluating the clinical application of the management pathway for hypertension.The results showed that the established management pathway can improve efficiency of health care workers,and the treatment and control rate of hypertension effectively.The hypertension management pathway provided guidance for health care workers and standardized the health care service behavior of medical staff enhancing the treatment and control rate of hypertension,which can provide a reference to achieve standardized management for other chronic disease.
