Adopting information technology makes the management of the chronic disease data quick and convenient and provides convenience declaring and evaluating. The paper also analyses the work mode of the system according to the business management person, the administrative person and the system manager respectively and the get the function modular. Thirdly, the detail of each function modular and the database design has been analyzed to implement the system.
