Background: With the development of social economy,the change of people's life style and the accelerated aging process,chronicdiseases,which are characterized by high morbidity,high mortality,high disability rate and long course of diseaseshave caused serious harm to human health and social economy.The contradiction between the uneven distribution of medical resources and the growing health needs ofpeople has led to low efficiency of the management of chronic diseases.With the rapid development of the information technologies,Internet of Things has been gradually applied inmedical field.Anew method of chronic disease management is being discovered.Purpose: Under the guidance of the bio-psycho-social medical model and according to the requirements of the national classification system,an effective,scientific and rational strategy of chronic disease health management in community based on Internet of things was established.The purpose of this strategy is to change management frompassiveto active,provide high-quality,continuous and fullservices for patients,promote the rational allocation of medical resources,standardizethe attendingsystem,relieve the attending pressure,and improvethe health service ability of primary medical institutions and doctors.Methods:To explorethese problems and challenges of chronic disease management,thepolicy environment and the experiences of chronic disease management domestic and abroad,literature analysis was used.From the perspective of diabetes and hypertension,astrategy of chronic disease health management in community based on Internet of Things was conceived.First of all,thispaperacquired the requirement specification of health management through semi-structured interviews,and established the specific management programs and strategies based on the demands and experiences.Then these programs and strategieswere applied to designing practices.Semi-structural interview was used to evaluated the management effect.Finally,this paper explored the problems and reasons of chronic disease health management by fishbone diagram method and put forward some suggestions.Results: This management information system was established to provide integratedmedical services which supported by hospital medical technology,based oncommunity health service centers,centered in service of patients and their families and used thesensing terminals.The management strategywas based on the combination of offline medical service and health management system platform,whichincluding health records management,assessment,intervention,dynamic tracking,counseling,education and two-way referral.Conclusions: Preliminary evaluation of this strategywaseffective.Most patients uploaded data as required,and were satisfied with sensing terminals.They actively followed the guidance of doctors and health management specialists including medicating,changinglifestyle,adjusting the state of minds and receiving health education.There was no quantitative management effect evaluation because of thetimelimitation.On the qualitative evaluation,there was a good reflection about the patients and medical staff's satisfaction evaluation.By the analysis of the causes of chronic disease management,this paper put forward some suggestions fromaspects of system standard,economy,personnel,management strategy and technology to promote the sustainable and effective development of chronic disease management.
