Design And Implementation Of Population Health And Chronic Disease Management Information System

Based on "information technology standards of chronic disease management business" of Bureau of Disease Control and Prevention of Ministry of Health and "standardized testing program about chronic disease management information system" and other relevant documents, A set of software organicly integrated with other management systems for chronic disease management is developped for the community health service centers and township health hospital and other primary health service.lt mainly is used to carry out the management of chronic non-communicable diseases by the hospitals of all or part of the business, and by "Regional Clinics", and by "Community Health Services" and by "Chronic Disease Prevention Station".The system consists of personal health records, chronic disease management, population information services, high blood pressure diagnosis and treatment systems, by collecting population-related information, the individual can be automatically screenned out to belong to the general population, chronic disease, or high-risk groups. Afterwards, through people classification management, the information of individuals further are collected such as the physical activity, diet, smoking, family history, laboratory tests and so on. The system will automatically generate all kinds of personal health assessment report, comprehensively assess their health status, and analyze the current situation and level of exposure of various risk factors, based on the information technology standards to management business of chronic disease, the health status of the chronic disease patients and the high-risk populations is assessed, and the individualized behavior intervention and mentoring programs to improve disease are formulated and regular follow-up, dynamic control and management of health status, on the basis of evaluation of the management effect, the behaviour intervention and treatment programs are adjusted by work together with the management objects, formating a course of dynamic cycle of track-assessment, guidance and management to achieve the three-pronged of treatment services and health education and track-managemen, and to reduce morbidity, complications,mortality and to improve quality of life of chronic disease patients.This paper, what based the theory on information management systems, combined with the actual needs, is developed under an advanced Microsoft.NET platform. Frontend using ASP.NET web interface design,backend database using SQL SERVER 2005 technology to do a deep research on designing the system. And after system analysis, platform architecture, database design, system design, and functional modular being design to develop a full support of the business. In the database design, the permissions to users, which base on their responsibilities could be distributed flexibly, to ensure the system operation safety and reliability. The system in practical application of the company shows that the study results saved manpower, material and time. avoided human error statistics maximally, simplified workflow, and improved work efficiency. The results can be obtained for other management system for reference.
