Chronic disease monitoring and management system is an emerging research, which benefited from the people's attention of the health management and the rapid development of network technology, not only has it the advantages of health management systems, but also has its own new features. Using chronic disease monitoring and management system, patients can break through the traditional mode of health care resources and treatment limitations, free from constraints of space and time, according to their progression to choose the best treatment time. Doctors can implement workflow processing of the treatment task, and can concern about the patient's condition in real-time analysis and dynamic tracing.This paper from the actual demand of the chronic disease monitoring and management, analyzes the model of the chronic disease monitoring and management system based on Web, and discusses in detail the main function of the system should have. Through the focus on the system's core issues, which include the monitoring data's uploading and the analysis management, proposed the file transfer mode based on the Web Services technology. For the analysis and management's problem of the data uploaded, proposed the problem's solutions, which stored in the database after parsing the file and let the program called Microsoft Excel to do statistical analysis, and taking into account the characteristics of drug history list's printing, propose a method to solve the problem better, means that the combined of the method of using stored procedures for database-side paging and the method of using dynamic binding on the page for client-side paging.On this basis, the background to the health management platform project's development and design, using B/S and C/S mixed mode, based on multi-tier architecture of .NET Framework, using the popular Web Services technology and SQL database, realized the chronic disease monitoring and management system based on Web. The main function of the current system has been achieved, and has a test run. feedback shows that the overall system design is reasonable, feasible measures to solve the problem.
