[Background] With the development of China’s economic construction and social progress, and on health concerns is deepening all the time. For health crowd, health and medical care have more demand and higher requirements. Currently, the residents of chronic disease has become the leading cause of death and health major health problem facing the crowd[1], was the focus of health care and core content.[Objective] Through the building of health population focused on chronic disease surveillance information system, we can make the chronic disease research early, clinical, continuous, digital, and ultimately, make it early detection, early diagnosis and early treatment of, improve health care efficiency, and improve the quality of health care.[Methods] By sorting medical records from several base hospitals for health crowd for people, exploring model monitoring of chronic diseases in health population,then we can establish the monitoring indicators of chronic disease in this crowd.[Result] Through the building of health population focused on chronic disease surveillance information system, explore the chronic disease monitoring business model suited to the characteristics of the health crowd, establish the monitoring indicators of chronic disease in this crowd.[Conclusion] The dynamic monitoring of chronic disease for the health crowd, health management and chronic disease monitoring methods combine the idea of, establishing a professional discipline common to study major diseases, sustainable improvement, a larger sample size of the database.
