ObjectiveBased on the competency model for nurses during the retired cadres of PLA’s transitional period from hospital to home, we established the transitional care nurse competency model, which helps to improve the nursing service system in transitional period and set up the basic training courses for transitional nurses.Methods1. Through literature review was performed in databases, we analysed periodical literature to refining competency elements of the transitional care nurse.2.Based on the assessment of Omaha System for nursing problems during the retired cadres of PLA’s transitional period from hospital to home, we analysed the contents of nursing services to refining competency elements of the transitional care nurse.3. Interview with the person who related to the retired cadres of PLA’s transitional period from hospital to home, we obtained competency elements of the transitional care nurse.4.Based on the competency elements extracted from the previous three studies, the construction of competency model for nurses during the retired cadres of PLA’s transitional period from hospital to home initially formed.5.Questionnaire survey was conducted 21 nursing specialists in the field of geriatric nursing, nursing management, nursing education and health management. After collected and analysed the valid questionnaires, we revised the transitional care nurse competency model.Results1. The results of literature review included 94 articles, 12 written in Chinese and 83 written in English. The key words in the transition period were extracted is 104, and the competency of the transitional care nurses which based on literature was found in the 14 aspects.2. Through the evaluation of nursing problems,81.25%(104/128) of retired cadres suffer from 4 or more diseases. The nursing problems involved four domains of Omaha System. Problems in physiological domain and health-related behaviors domain were the most problematic. Based on 30 nursing problems during the retired cadres of PL A’s transitional period from hospital to home, we analysed the contents of nursing services to refining 19 competency elements of the transitional care nurse.3. Interview with the person who related to the retired cadres of PLA’s transitional period from hospital to home, we obtained 37 competency elements of the transitional care nurse.4.Based on the competency elements extracted from the previous three studies, the construction of transitional care nurse competency model, including 5 first-level indicators, 18 secondary-level indicators and 29 competency elements.5. Through 3 rounds of expert consultation, the competency model for nurses during the retired cadres of PLA’s transitional period from hospital to home,including 4 first-level indicators, 14 secondary-level indicators and 42 competency elements. The weights of the indicators and elements are calculated.ConclusionBased on literature reviewe, the evaluation of nursing problems, semi-structured interviews and experts enquiry, we developed the transitional care nurse competency model, which has the reference value for the training of nurses in transition period, the improvement of nursing service system and the development of relevant service standards. |
