Study On The Definition Of The Responsibilities Of Nurse Anesthetists During Perianaesthesia Period

Objective:The objective of this study was to establish the responsibilities with the actual situation in our country and its weights of nurses anesthetists during perianaesthesia period by using Delphi and other methods,and to carry out the empirical analysis.Also,to determine the perianaesthesia work responsibilities and related requirements of nurse anesthetists,to provide reference for better cooperation with anesthesiologists in the completion of clinical anesthesia work,and to provide basis for training specialized anesthetic nurses with Chinese characteristics in future countries and its qualification certification.Methods:On the basis of previous literature research,expert semi-structured interview,actual investigation and pre-investigation,the author first drew up the draft of the index.Then,using the self-designed expert questionnaire to consult with 25 experts selected on index items and their connotation.The experts' opinions and results were sorted out,statistically analyzed to determine the responsibilities of the nurse anesthetists during the period of perianaesthesia and the weight coefficient of each index.According to the work responsibility index,the questionnaire survey of nurses' perianaesthesia duty was carried out.279 nurses in anesthetic department in some second and third level hospitals were analyzed to verify the reliability and validity of the index by the empirical analysis,and finally to determine the job responsibility.In statistical processing,Excel2016 and SPSS22.0 were used to describe the composition structure of consulting experts,and the indexes such as the degree of concentration of expert opinion,the degree of coordination,the positive degree and the authority coefficient are used.In the part of the empirical analysis of nurses' perianaesthesia duty,the reliability and validity of the index were evaluated by Amossoftware.The reliability of the index was tested by the Cronbach's ? coefficient and the structural validity was verified by correlation analysis.Results:1.The results of the expert consultation has high reliability and authority.The study selected 25 experts from 10 hospitals in China,including Beijing,Shanghai,Fujian,Jiangsu,Xi'an and other cities,who were engaged in clinical anaesthesia,anesthetic nursing and nursing management to carry out Delphi expert consultation.Two rounds of Delphi expert consultation,experts degree of authority coefficient of was0.913,and the effective recovery rate were 100%,indicating that the experts have a higher enthusiasm for this study.The results of the second round of expert consultation showed that the coordination coefficient of the third-level index was 0.245?0.270?0.219(P < 0.001),which was higher than that of the first round.2.The results of the Delphi method for determination of the work responsibilities of nurse anesthetists during perianaesthesia period.Likert scale was used to assign the index.According to the criteria of index selection,namely,the mean value of the importance of each index is > 4.0,the coefficient of variation < 0.25.Combined with expert opinion and the discussion of the research group,the index was revised.After two rounds of expert consultation,the indexes of nurses' work duty in anesthetic department during the period of anesthesia were determined,including 3first-level indexes,16 second-level indexes and 82 third-level indexes.3.The results of the weights of the indexes of nurses' perianaesthesia duty.Based on the evaluation of the relative importance of the indexes by the experts in the second round of Delphi,the weight of the indexes is determined,which reflects the relative importance of the indexes in the whole index system.Finally,the first index weight was determined: basic quality(0.340),perianaesthesia work responsibility(0.340),professional development ability(0.320),and other index weight distribution at all levels.4.The empirical analysis of the perianaesthesia duty of nurse anesthetists.The reliability and validity of the work duties of nurses in anesthesiology department were good.SPSS22.0 software analysis showed that the Cronbach's ?of the whole index was 0.971,the Cronbach's ? of the first-level index wasbetween 0.894 and 0.955,Cronbach's ? of the second-level index was between0.702 and 0.940,the correlation coefficient between the first-level indexes and all levels of indexes is between 0.554 and 0.955,and the P-value is less than 0.05.Conclusion:At present,the anaesthetic nursing work in some hospitals in our country has been carried out by the nurse anesthetists,and it is very professional,but there is no unified job responsibility in the whole country.In the development of specialized nurses in our country,the nurse anesthetists are not listed among them.But its development momentum is strong.Nurse anesthetists are bound to be an important branch of specialized nurses in our country in the future,their work responsibilities need to be clearly defined.In this study,the experts of clinical anesthesiology and anesthetic nursing specialists in China were consulted.The main duties of nurse anesthetists during the period of perianaesthesia were defined,and the indexes formed were scientific and reliable.The contents of nurses' work in anesthetic department at present in our country were understood through actual measurement.It provides a reference for the cultivation of the anesthesia specialist nurse and the development of anesthetic nursing in the future.
