[Objectives] 1.To establish a set of initial accreditation and evaluation index of transitional care specialist nurse.2.To establish the weight coefficient of each index,so as to achieve a quantitative initial accreditation and evaluation index.[Method] In this study,the preliminary establishment of index was based on literature review,group discussion and theoretical analysis.The initial index was sifted and modified after two rounds of questionnaires from 24 experts according to the Delphi method.The credibility and representativeness of the Delphi method was tested by calculating the positive coefficient,the authority coefficient and the coordination coefficient of experts.On the basis of the Delphi method,we also applied the analytic hierarchy process to determine the weight coefficient of each index.[Results] 1.Using literature review,group discussion,theoretical analysis and the Delphi method,we developed the index including 6 level-one indices,24 level-two indices and 93 level-three indices.2.Experts involved in the Delphi method are from five different provinces,autonomous regions and special administrative regions,which means an acceptable representativeness of regions and academy.Questionnaires' recovery rates in two rounds of expert consultation were 89.7% and 87.5% respectively,effective recovery rates were 92.3% and 100% respectively,and opinion submission rates were 58.3% and 47.6% respectively,indicating experts' concern and motivation of this research.The authority coefficient in two rounds of expert consultation were 0.889 and 0.892 respectively,reflecting the good credibility of the Delphi method.The Kendall coefficients of level-one indices,level-two indices and level-three indices in two rounds of expert consultation were all between 0 and 1,and all of them had statistical significance after testing,implying that opinions of experts were well coordinated.3.The analytic hierarchy process was applied to determine the weight coefficient of each index.Level-one indices were ranked according to the weight coefficient: practical experience = 0.2535,core competence = 0.2535,professional attitude = 0.1715,access conditions = 0.1379,theoretical learning = 0.1099,certification exam(?80 points)= 0.0738.The weight of level-two and level-three indices were calculated by the same method.[Conclusion] 1.In this study,experts involved in the Delphi method are highly authoritative and have concentrated opinions,and the results of the consultation have high credibility.2.The initial accreditation and evaluation index of transitional care specialist nurse we established covers 6 aspects and has 123 indices in total,including the access conditions,the occupational attitude,the theoretical learning,the practical experience,the certification exam(?80 points)and the core competence.3.The weight coefficient of each index is established through the analytic hierarchy process.Among them,the largest two are " the practical experience" and " the core competence",and the rest are "the occupational attitude","the access conditions","the theoretical learning",and "the certification exam(?80 points)" in order of the weight coefficient.
