Objective:This study was commissioned by the National Health and Family Planning Commission's Talent Exchange Center.Based on the competency model of head nurse formed in the early stage,combined with the latest competency study,this study developed the head nurse competency evaluation questionnaire suitable for the current situation of our country,and set up the weight,verified the reliability and validity of the questionnaire by the application of the questionnaire in 3A hospital in China to form a more reasonable competency assessment tool for head nurse.Besides,using the developed questionnaire to conduct a large sample investigation in 3A hospital nationwide,we can understand the current situation and ability demand of the competency of head nurse.The related factors are discussed in order to provide a reference for the development of training programs for head nurse.Methods:1.Through literature review and discussion among the members of the project group,the dimensions and items of the head nurse competency evaluation questionnaire were preliminarily drawn up.2.Development process of questionnaire:(1)After a two-round Delphi consultation,the questionnaire dimensions and items were added or subtracted,integrated and revised by calculating the degree of concentration of expert opinion.Meanwhile,the reliability and representativeness of the experts were investigated by calculating the positive coefficient of experts,the degree of authority and the coordination coefficient.(2)Using analytic hierarchy process(AHP)to set the weight of the questionnaire dimensions and items,then the head nurse competency evaluation questionnaire is perfected.(3)The reliability and validity of the questionnaire were verified with a small sample survey in 6 grade 3A hospitals in China,and on this basis the competency questionnaire of head nurse in China was formed.3.Using the method of questionnaire,the head nurse of 22 Grade 3A hospitals in our country were investigated with large sample,and the frequency and percentage were used to analyze the general data of the subjects.In the form of mean standard deviation,this paper describes the current situation of head nurse'competency score,and discusses the related factors of influencing head nurse'competency scores by means of t-test or variance analysis and multiple stepwise regression analysis.Results:1.In this study,a 6-dimension and 28-item of Chinese head nurse competency questionnaire was formed.2.Results of questionnaire development:(1)The positive coefficients of two rounds of consultation were 100%and 78%,respectively.The authority coefficient of the expert group was 0.841,the coordination degree of the second round of expert opinion was0.4073,and the item was 0.5066,which was much higher than the first round(P<0.05).(2)In analytic hierarchy process(AHP),the questionnaire dimensions weight from high to low were cognitive characteristic(0.2855),management characteristic(0.2266),personal efficacy(0.1799),help and service characteristic(0.1334),achievement and action characteristic(0.1059),impact and influence characteristic(0.0686).(3)Pre-survey data analysis showed that the overall Cronbach's Alpha coefficient of the questionnaire was0.993,and the coefficient of the six dimensions was 0.925~0.973.In the correlation analysis,the correlation coefficient between each dimension of the questionnaire and the total questionnaire was 0.914~0.991,the difference was statistically significant(P<0.05).In the confirmatory factor analysis,?~2/df=3.324,the values of CFI and TLI were both above 0.95,the confidence interval of 90%was in the range of accurate fitting,so the fitting degree of this model was acceptable.Therefore,the reliability and validity of the questionnaire were good.3.Current investigation results:(1)The total mean score of head nurse competency in China is(125.46±16.562),which reached the total score of more than 80%,indicating that the overall level of competency of head nurse was above average.(2)In terms of age,personnel status,professional and technical titles,the highest educational background,years of primary nursing work,the working years of nursing management,the number of times of training in the past year and the region,the difference of competency and total score of head nurse were statistically significant(P<0.05).(3)Multiple stepwise regression analysis showed that the main factors influencing the competency of head nurse were the highest academic qualifications,the years of primary nursing work and nursing management years.Conclusion:1.In this study,a competency questionnaire for head nurse with 6 dimensions and 28items was developed,which was scientifically rigorous,authoritative and targeted.The questionnaire was verified to have good reliability and validity by a national small scale pre-survey,which provides a scientific and reasonable tool for evaluating the competence of head nurse,and meanwhile provides the basis for selecting,training and evaluation of head nurse.2.At present,the general level of competency of head nurse in the 3A hospital is relatively high,but the score of cognitive characteristic is relatively low.Further improvement is needed in the head nurse'scientific research thinking and teaching ability.At the same time,the main factors affecting the competence of head nurse are the highest educational level,the years of primary nursing work and nursing management,we should pay attention to the training of head nurse who are engaged in primary nursing work and nursing management work for a short period of time,and give more attention and support to nurse who have low overall competence.
