Establishment Of Competency Evaluation Index System For Nurses In Cardiac Rehabilitation Specialty

Objective: The purpose of this study is to construct a set of competency evaluation index system for the post of cardiac rehabilitation nurses conforming to the national conditions of China,so as to provide target guidance for the training of cardiac rehabilitation nurses and basis for the training and assessment of cardiac rehabilitation nurses,so as to promote the rapid development of cardiac rehabilitation nursing in China.Methods: This study is a mixed study,including descriptive nature and Delphi method.1.Literature analysis: By referring to the relevant literature of cardiac rehabilitation nurses at home and abroad,the competency elements of cardiac rehabilitation nurses were analyzed and extracted by content analysis method.2.Qualitative interview: Through objective sampling and following the principle of difference maximization,doctors,nursing managers,nurses,rehabilitation therapists and patients from cardiovascular surgery/rehabilitation/cardiac rehabilitation(outpatient)department were selected for semi-structured interview.Content analysis method was adopted for data analysis,and the competency evaluation index of nurses in the specialty of cardiac rehabilitation was preliminarily constructed.3.Delphi method: Conduct three rounds of expert correspondence consultation on wechat,email or face to face for 16 experts who meet the selection criteria,and use Excel2016 and SPSS 24.0 software for data statistics and analysis.The reliability of the three rounds of expert letter consultation results was tested according to the positive coefficient of experts,the degree of authority of experts,the degree of concentration of experts’ opinions and the degree of coordination of experts’ opinions,and finally the competency evaluation index system of nurses specialized in cardiac rehabilitation was formed.4.Analytic hierarchy process(AHP): According to the mean difference of the importance score of the competency evaluation index of cardiac rehabilitation nurses in the last round of expert letter consultation,the AHP software yaahp11.0 was used to calculate the weight and combined weight of each evaluation index.Results: The competency evaluation index system of cardiac rehabilitation nurses includes 5 first-level indexes,30 second-level indexes and 92 third-level indexes.The effective recovery rate of the first two rounds of expert letter consultation questionnaire was 100%,and the expert authority coefficient was 0.935.The effective recovery rate of the third round of expert letter consultation questionnaire was 93.75%,and the expert authority coefficient was 0.930.The Kendall coordination coefficient of the three rounds of letter consultation were 0.290,0.272,and 0.297,respectively.The index weights were determined by AHP.Among the knowledge indexes,the second-level indexes with the highest weights were cardiovascular disease knowledge and sports rehabilitation knowledge,and the weight coefficients were both 0.2370.Among the skill indexes,the secondary indexes with the highest weight were cardiovascular skill and cardiac rehabilitation skill,with the weight coefficient of 0.2237.Among the personality traits,the second-level index with the highest weight was conscientiousness,with the weight coefficient of 0.3731.Among the motivation indicators,the second-level indicator with the highest weight is independent learning,and the weight coefficient is 0.6554.Among the self-concept indicators,the second-level index with the highest weight is professional attitude,and the weight coefficient is 0.7500.Conclusion: The competency evaluation index system of nurses specialized in cardiac rehabilitation established in this study is scientific,reliable,comprehensive and practical,and can provide a basis for the cultivation,training and assessment of nurses specialized in cardiac rehabilitation in China.
