Chronic disease management information system is designed, based on the chronic disease control and management of health service organizations in community health service centers and in township, according to the relevant provisions about chronic disease management business information technology standard and chronic disease testing standard of health disease prevention and control center in the National Health Ministry, combined with the development and design of community management information system, the popularization and application of chronic disease management information system software in the community health service institutions, regional outpatient department played a very good use.In this thesis, based on the management information system theory, according to the oriented-object design principle, the development and design of system is supported on the Microsoft.NET platform. SQL SERVER 2008 database is used, interface design is beautiful and applicative, ASP.NET technology is used to connect the network, the system architecture design, system function analysis, database structure design, system overall design, detailed design of each functional module is included in the paper, the establishment of personal health records, health information service, diagnosis and treatment of hypertension, information processing of chronic diseases and other functions can be completed with the information system management of chronic disease. after inputting patient information, the disease range can be automaticly screened, and then personal and family disease history, diet, daily physical security, and other information can be collected, the personal health assessment can automatically completed with the system, the potential risk disease symptom is analyzed, according to the national chronic disease management information requirements, the archives of patients with chronic disease is established the guide of the prevention and treatment measures is set up, chronic disease and population health are dynamicly monitored, the patients with chronic disease are visited and examinated, the effective management information system is achieved the public health prevention, protection, tracking and processing combined, the disease incidence and disease complications being reduced,the mortality rate being reduced, and the life quality of the patients with chronic diseases is effectively improvedThe security of the system is effectively guaranteed by using the permissions setup on the client, the system running environment is safe and reliable. With design and development of chronic disease management information system, the work efficiency and accuracy of data processing are improved, working procedures are simplified, the error rate of artificial processing is reduced, the research and development of the chronic disease management information system can be used as a design reference for similar system.
