In recent years, a variety of social, economic and environmental factors like the increasing aging population, changing lifestyle, causes the prevalence of chronic diseases is increasing.260million people with chronic diseases have become a major hazard of the national health, which not only brings economic pressure and ideological burden to patients and their family but also creates a challenge for the entire health care system and social service system. Long-term monitoring and treatment, which is needed to the chronic patients because chronic disease has characterized longer duration, poor prognosis and a high incidence of complications, can’t be received by the patients since the lack of medical resources and the limit of the patients’ family economic. In order to change this situation, the chronic disease prevention and control center of the existing mode is in urgent need to shift from central hospital to community health service stations and family. Meanwhile, the medical model should also be change from "treatment based, prevention supplemented" to "Prevent sickness", which focuses on prevention and control.In patients with chronic diseased without affecting the normal work and do not reduce their quality of life, currently, is the key and difficult of chronic disease prevention and control. Take this as a research point, a chronic health management system which can be used in our everyday lives and work has been developed. It meets the needs of patients with chronic diseases and health interventions for routine care, but also achieves the purpose of scientific management of chronic disease in order to prevent and control chronic diseases. The system consists of three parts, namely, miniature multi-parameter health monitor, health-mobile phones and health management server.The development of mobile healthcare is promoted by the rapid development of sensors, communication networks, electronics and other technology. Thereby, a new mobile medical monitoring method is available by the mobile intelligent terminal which has flexible function customization, powerful computing capability and large user community. In this paper, we focus on the design and implementation of health monitoring and management system on smart phone, which achieves health care management function through gathering physiological data from health-care receive via Bluetooth wireless technology for monitoring and sending data to health management server by the network for further processing.By using the early portable monitoring system, such as Holter system, telephone-based (BP machine-based) or PC-based monitoring system, although the monitor of patient’s physiological indicators is no longer restricted in hospital, but It is difficult to meet the purpose that monitoring without affecting the user’s normal work and life precondition while Reading of information relies on specific instrument, limited amount of information processing or restricted range of activities. Proposed the concept of a wearable monitoring, they quickly get a lot of attention, and numerous researchers started studying health care based on wearable devices. In recent years, with the rapid development of mobile intelligent terminals and mobile communication technology, health care systems based on3G/4G networks and mobile intelligent terminal has become a hot field and the a direction of future development of medical care.Based on the analysis of the existing smartphone operating system, choose the Android and iOS system, which has a higher market share, more mature development technology, as a development platform for mobile health monitoring and management system. As the middle layer of the whole health monitoring and management system, smart phone need to fulfill the task of "transmission", namely, receive physiological data from multi-parameter health monitor and exchange data with the health management server. By analyzed health monitoring and health management requirements of chronic disease, the system main function can be divided into health monitoring, personal information management and health information management three parts, and health monitoring can be decomposed into the Bluetooth data communication, monitoring data acquisition and display. A Bluetooth data transmission protocol which be followed between the health monitor and mobile phone was defined to ensure the stability and security of the data transmission. Two applications were implemented with characteristics of Android and iOS. The function of the Android application implementation have personal information, health log, daily monitoring, diagnosis, monitoring, interactive platform, appointment and health site. The personal information management, health monitoring, health information management and fall detection were achieved in the iOS application.Chronic patients have a higher incidence of falls. Timely fall detection and treatment can reduce the extent of injury or even reduce the incidence of death. Fall detect algorithm, implemented on iPhone, extract characteristic value related to human movement, intensity and Angle acceleration from3D acceleration and the change of Angle of human body which is collected by utilizing iPhone’s built-in3D accelerometer and gyroscope, then, multi-threshold judgment method was used to determine whether a fall occurs. Large of data was collected in ADL (activity daily life) and falls. The threshold value of threshold determination algorithm is confirmed through analysis and processing MATLAB. Through real falls and daily activities events, we detect the rate of false positives and misstatement rate of the algorithm. The experiments show that the algorithm achieves99.25%correct, that means it can effectively detect fall.For the patients with chronic and other people to pursue a healthy life, this study designed mobile health monitoring and management system and on Android and iOS, respectively, to achieve the application. Research and formulate the Bluetooth data protocol which was followed by the communication between the mobile phone and multi-parameter health monitoring terminal; The system realized the real-time monitoring of heart rate, oxygen saturation and ECG which based on physiological parameters provided by the monitoring terminal, stored and managed the physiological parameters, and thus provide an effective basis for the user’s state of health; Using the iPhone’s built-in three-axis accelerometer and gyroscope to obtain the eigenvalues of human movement, and to achieve the human fall detection algorithm through determine thresholds. But there are still some aspects in need of improvement in this study, such as the robustness and stability of the application and the user experience should be further improved; Function of the client should be combined with the use of user feedback for further modification and strengthen; To improve the accuracy of the falling detection algorithm, more real data is needed. With the development and improvement of mobile health monitoring and management system and other system modules, health monitoring and management system will provide an effective health management system for chronic diseases.
