Objective:(1) To observe the general effect score, histopathology and hemorheology changes of the treatment by Seven Pinches Powder combined with Mirabilite to acute soft tissue injury of guinea pig;(2) To observe the clinical effect of the treatment by Seven Pinches Powder combined with Mirabilite to initial swelling after fracture, explore the mechanism, and provide theoretical and clinical basis for the promotion of Seven Pinches Powder combined with Mirabilite.Methods:(1) Take50guinea pigs which are female,302±4.36g, ordinary level. and randomizedly evenly divided into blank group, model group, Seven Pinches Powder group, Mirabilite group, combined group (treated by Seven Pinches Powder combined with Mirabilite group). According to refrences acute soft tissue injury guinea pig model was established by iron pestle striked on the right lower limb soft tissue of guinea pigs from each group except blank group. In different group, different drugs were applied on the injury spots except the model group and blank group,7days for treatmen. A general observation was done on day1, day3and day7after application. Then by determination of heart blood hemorheology after general anesthetic, pathological section of subcutaneous tissue cut from the soft wounded tissue, observation and comparison under microscope after putting to death on day7after application.(2)65patients were included in the trial who had been diagnosed trauma-induced swelling in middle of tibia and fibula closed fracture, finally, there were58patients up to the end of trial, the treatment group32patients and the control group26patients. the control group:calcaneus traction and20%mannitol125ml for intravenous infusion everytime,2times per day, oral pain tablets,1/time,1time/day; the treatment group:calcaneus traction and20%mannitol125ml for intravenous infusion everytime, two times per day, at the same time, Seven Pinches Powder combined with Mirabilite was applied externally at the local swollen skin, keep the external application time3hours each time.2times per day.7days for treatmen. Measure the circumference of both the affected and the contralateral side at9:00and21:00each day, then calculate the circumference difference. Through comparing the new observation with last one, record the time when swelling in the most severe case. Compare the circumference difference, ecchymosis score and pain score between the two groups before treatment and on day2. day3. day7after treatment. Observe the topical skin reaction, check the blood, liver and kidney function after the test. All data were processed with SPSS16.0software statistics. Results:(1)1. The general effect score:On day1after application, between combined group and model group, Seven Pinches Powder group, Mirabilite group, the general effect score was no statistical difference (P>0.5); but on day3and day7it was significantly statistical difference between combined group and model group,Mirabilite group (P<0.01), it was statistical difference between combined group and Seven Pinches Powder group(P<0.05);2. Pathological section score was statistical difference between combined group and model group, Mirabilite group(P<0.05), but it was no statistical difference between combined group and Seven Pinches Powder group (P>0.5);3. Hemorheology:Whole blood viscosity, plasma viscosity and hematocrit were significantly statistical differences between combined group and model group (P<0.01); whole blood viscosity was significantly statistical difference between combined group and Seven Pinches Powder group (P<0.01), plasma viscosity and hematocrit were statistical differences(P<0.05); whole blood high shear viscosity and whole blood low shear viscosity were significantly statistical differences between combined group and Mirabilite group (P<0.01), plasma viscosity and hematocrit were statistical differences(P<0.05), whole blood medium shear viscosity was no statistical difference (P>0.5).(2) No patient of treatment group had allergic skin reaction. Before treatmen and on day2after treatment, the circumference difference and ecchymosis score between the two groups were no statistical differences (P>0.5),but on day3they were statistical differences(P<0.05) and on day7they were significantly statistical differences (P <0.01). Before treatmen, on day2and day3after treatment the pain score was no statistical difference between the two groups (P>0.5), but on day7it was statistical difference (P<0.05); between on day7after treatment and before treatmen, the circumference difference in treatment group was significantly statistical difference (P<0.01) and it was no statistical difference in control group, ecchymosis score and pain score were significantly statistical differences in two groups (P<0.01).No patient of Two groups had blood, liver and kidney function abnormalities.Conclusion:Seven Pinches Powder combined with Mirabilite can significantly promote initial swelling subsiding,the absorption of blood stasis and alleviate pain of acute soft tissue injury. It has positive meaning for the treatment of initial soft tissue swelling after fracture and may be worthy of wide promotion and application in clinical.
