Study On The Index Framework For Application Of Grade Duty Nursing Pattern In Good Quality Demonstration Ward In Zhejiang Province

ObjectivesThe aim of this study was to design grade levels, access conditions, training content, job responsibilities for good quality demonstration ward nurses at all levels in Grade-ⅡA and higher Hospitals, Zhejiang Province, to provide referential suggestions for the establishment of a mature nurse management system.MethodsBased on reviewing on a great deal of domestic and foreign literature, and based on theoretical guidance of the Fit of Capacity and Position Model, this study was full use of purposive sampling for interview with directors of nursing department, head nurses, senior nurses and responsible nurses, refining and screening different grade indexes, and combining with Zhejiang situation, to design a draft of index framework. Directors of Quality Control Centers and experts with experienced nursing management in representative areas all over the province were selected from expert database of Zhejiang Province Nursing Center, Delphi Method was used, in accordance with 5 grade Likert scale method, to evaluate the importance and feasibility of the draft indexes. The SPSS 16.0 was used to analyze experts'positive coefficient, authority coefficient, variation coefficient, and coordination coefficient.ResultsIn the first round of expert consultation, positive coefficient:recovery rate was 95.3%, proposal rate was 70.7%, authority coefficient was 0.839; for index importance, its average score was 4.34-5.00, its variation coefficient was 0.00-0.21; for index feasibility, its average score was 3.93-4.93, its variation coefficient was 0.05-0.30; coordination coefficient of index importance was 0.084; coordination coefficient of index feasibility was 0.075. In second round of expert consultation, positive coefficient: recovery rate was 97.6%, proposal rate was 25%, authority coefficient was 0.840; for index importance, its average score was 4.58-5.00, its variation coefficient was 0.00-0.17; for index feasibility, its average score was 4.21-4.93, its variation coefficient was 0.00-0.30; coordination coefficient of index importance was 0.061; coordination coefficient of index feasibility was 0.065. After two rounds of expert consultation, the index framework was established for application of grade duty nursing pattern in good quality demonstration ward in Zhejiang Province. Nurses are graded four levels:nursing group leader, senior responsible nurse, junior responsible nurse and assistant nurse. There are relative access conditions, training content, job responsibilities for nurses in all grade levels.ConclusionsThe index framework was set up, which included grade levels, access conditions, training content, job responsibilities for good quality demonstration ward nurses at all levels in Zhejiang Province. It had good opinion centrality. The results showed that experts' consulting suggestions had high representativeness and dependability.
