Construction Of The Evaluation Index Framework Of Nursing Care Transfer Quality In ICU Transition Period

Objective:This study aims to clarify the connotation of nursing care transfer,combined with literature analysis,to initially construct a transitional nursing care transfer quality evaluation indicator framework for ICU patients,and conduct reliability and validity tests,so as to help nursing managers and clinical nurses to estimate and monitor the quality of nursing care transfer,and provide a theoretical basis for realizing the scientific and standardized management of nursing quality.Methods:1.Combining literature analysis and semi-structured interviews,with the structure-process-result three-dimensional quality management as the basis,the item pool of the evaluation index is initially constructed.2.Through two rounds of Delphi expert letter inquiries(Delphi),the indicator is repeatedly screened and modified,and the letter inquiries can be closed until the expert opinions are reached.3.Use Analytic Hierarchy Process(AHP)to calculate the weight of each indicator and the combined weight.4.Compile a questionnaire based on the initially constructed evaluation indicators,select 324 ICU nurses and 8 clinical nursing experts in four tertiary A hospitals in Shanxi Province to test the reliability and validity of the indicator system,using the critical ratio method to screen out meaningful items,and the Pearson correlation analysis is used to test the correlation between items,and using Cronbach’s α coefficient,split-half reliability and exploratory factor analysis to test the reliability and validity of the index system.Results:1.Preliminary establishment of an evaluation index framework for the quality of nursing care transfer during the transition period of ICU.A total of 26 experts in this study participated in the completion of two rounds of letter inquiries.The experts are highly motivated and authoritative,and the effective recovery rates of the 2 rounds of expert letter questionnaires in this study were 92.9% and84.6%,the experts’ authority coefficients were 0.824 and 0.907,and the opinions on each indicator are more consistent,the Kendall harmony coefficients of expert opinions were0.224 and 0.245.Finally,an index framework for evaluating the quality of nursing care transfer in the ICU patients was constructed,including 3 first-level indicators,15second-level indicators and 47 third-level indicators.2.Determining the weights of all levels of indicators in the ICU transitional period of nursing care transfer quality evaluation indicators.In this study,the analytic hierarchy process was used to calculate the weights of all levels of indicators through Yahhp software.In the first level indicators,the weights of process and result indicators are the same,both are 0.4286,and the weight of structural indicators is 0.1429.3.Reliability and Validity Test of the Evaluation Index of Nursing Care Transfer Quality in ICU Transition Period.In this study,the project analysis results showed that the decision value of all items was greater than 3.0,so all items were retained.In the Pearson correlation analysis,the correlation coefficient(r)between each dimension and the total score is between0.715-0.956,and the correlation is significant(P<0.01),so the correlation between each dimension is better.According to the results of reliability analysis in this study,the Cronbach’s αcoefficient of the total index system is 0.957,and the half-half coefficient is 0.802.The Cronbach’s α coefficient of each dimension in the indicator system is between 0.966 and0.978,and the half-half coefficient of each dimension is between 0.892 and 0.985.Both are greater than 0.8,so the index system has good reliability.In the validity analysis,the content validity index S-CVI is 0.976.In the exploratory factor analysis,the characteristic root>1.5 was used as the standard,and 3 common factors were extracted.The cumulative contribution rate of the variance reached 69.236%,so the index system has good validity.Conclusion:The content of the index framework constructed in this study is true and reliable,the method is rigorous and reasonable,and the results are scientific and credible.It can reflect the core connotation and basic points of transitional nursing care transfer for ICU patients in multiple dimensions,and can be used to provide a scientific basis for the quality evaluation of nursing care transfer.
