Urban community health service is an important part of health work and it is the basic link for realization of goals of primary health care for the entire. Community health information is tools and instruments of community health management. Accelerate the development of community health services, good construction and management of information is one of the key parts, and meanwhile it is also an important foundation for sustainable development to promote community health services. With the depth of community health services' development, public health needs are increasing. To establish a scientific, comprehensive community health information management system, to accelerate construction of community health service system, and to achieve community health management of modern business have been very important.Resident health archive is an important function of community service which is different from the second or third grade hospitals and the focus of the community health service. Dynamic information is the most essential part of health archives. The paper designed and implemented the urban and country resident health archives and the community diagnostic and therapeutic practice information system which combine clinical therapy and public health management through the analysis of urban community health service practice and procedure as well as the real demand for Weifang City as a pilot one. And the following results have been achieved:1. Realize the positive dynamic archive management. Health information can be collected and perfected through every health examination, outpatient service as well as in-hospital service for the residents. Then break the traditional isolated and static health archive management mode; and form an integrated, dynamic electronic health information warehouse.2. Implement the effective management for the chronic disease in public health. Through the dynamic supervision and functional analysis of the main diagnosis index of the patient with chronic diseases, visit timely and provide early-warning information and control the disease development effectively.
