A Hypertensive Community Management System Based On Mobile Health

Hypertension is a common chronic disease.According to statistics,about 1 billion people in the world are affected by high blood pressure.The prevalence of hypertension in our country over 18 years old is as high as 25.2%.National guidelines for prevention and treatment of primary hypertension in 2017 pointed out that in our country,grass-roots medical and health institutions are the main battlefield for hypertension management.The level of its management will directly affect the future development trend of cardiovascular and cerebrovascular diseases in our country.Long-term experiments both at home and abroad have proved that the most effective way to increase blood pressure control rate is community control.The development of mobile medical treatment also provides convenient help for us to solve the hypertension management problem.This dissertation summarizes the current main problems of community management and current mobile medical software is not ideal for controlling the rate of hypertension,based on the 2017 national guidelines for prevention and treatment of primary hypertension,designed and developed a community-based hypertension management system.Patients can select their own health specialists on their APP based on ratings and other criteria.The health specialist will release personalized reports to patients on request,including physical measurements,diet,exercise,and health education.Patients receive tasks in real time and set reminders to see compliance rates and goals achieved.If patients have any doubts about the plan,they can communicate with the health professional through the built-in chat module of the APP and adjust the plan.Through the role of the health commissioner,the system can develop a plan for patients in all directions,and according to the risk assessment model of hypertension to regularly screen high-risk patients,help to improve the level of control of hypertension and reduce the complications of hypertension.
