| To observe the syndrome characteristics of the gout arthritis in the acute attack period and the intermission. Under the opinion of'Blood heated boiling' and the principle of'relieving the secondary in an urgent case'and'purging excess', we use'Qingreliangxuefang'to cure the patients with gouty arthritis. Then we observe the near and future curable effect, and the safety of the prescription.1 Grouping the study objectives65 sufferers grouped in two teams:Study group which has 32 sufferers is cured with both western medicine and Chinese traditional medicine. Their ages are between 52.34±15.69 years. Their course of disease is 41.25±24.66 months. Control group which has 33 sufferers is cured with western medicine only. Their ages are between 53.45±13.61 years. Their course of disease is 43.00±25.20 months.When the two group patients joined in the study, personal ill histories, compassion illnesses and basic character, we didn't find statistic sense. The diagnosis accorded with the Academy of rheumatology's standard. And all of them belonged to original gout.2 therapeutic methodBoth groups must keep low purine diet in the treatment. The study group: In the acute attack period, patients were treated with'Qingreliangxuefan-g'once perday and colchicine 0.5mg per 2 hours once, and the total was less than 6mg. Both the herbal medicine and colchicine were taked orally. The colchicine stoped when patients felt pain come better. Then patients kept taking the herbal medicine, at the same time, took allopurinol orally,50mg once,3 times perday. The control group:In the acute attack period, patients were treated with colchicine by mouth,0.5mg per 2 hours once, and the total was less than 6mg. The colchicine stoped when patients felt pain come better. Then patients were treated with taking allopurinol orally,50 mg once, 3 times perday. The two groups were treated for 4 weeks.3 Examination methodThe examinations of blood uric acid (UA), erythrocyte sedimentation rate (ESR), white blood cell (WBC), C reactive protein (CRP), hepatic and renal functions have been done before, in and after the treatment. HITACHI 7020 Automatic Analyzer was used to examine their blood uric acid, C reactive protein, hepatic and renal functions. NIHON KOHDEN MEK-6318K was used to take account of the numbers of white blood cells.4 Look-in indexWe evaluated the curative effect by the patients'clinic symptom improvement, all kinds of experiment index improvement, the time of urgent gout arthritis came on. And we took security in account and entirely evaluated its clinic effect.5 Statistic methodWe used chi-square test to know the enumeration data distribution, used (x±s) and t-test to know the clinic measurement data distribution. And we used rank-test to know the curative effect.The study group has shorter time to reduce patients'pain than the control group in the acute attack period. After one week treatment, the study group got 25 markedly improvement,6 effective and 1 failure cases. The total effective rate is 96.8%. The control group got 11 markedly improvement,16 effective and 6 failure cases. The total effective rate is 81.8%. So the curative effect of the study group is obviously better than the control group (P<0.01). Four weeks after treatment, the study group got 29 clinical recovery,1 markedly improvement,2 effective, and no failure cases. The total effective rate is 100%. The control group got 16 clinical recovery,9 markedly improvement,5 effective and 3 failure cases. The total effective rate is 90.9%. So the curative effect of the study group is obviously better than the control group (P<0.01). One week after treatment, all the laboratory parameters of the study group got lower than before (P<0.01), and to compare with the control group, the parameters except UA and WBC(P>0.05) got lower, too (P<0.01). ESR of the control group was no significant difference in the first week(P>0.05), but other parameters (UA, CRP, WBC) changed obviously than before (P<0.01). So it shows that the study group has better effect on acute attack than the control group (P<0.01). After four weeks treatment, the laboratory parameters belong to both groups were improved than before treatment (P<0.01), and the study group's got lower (P<0.01). It shows that although the two groups had effect after 4-week treatment, the efficacy of the study group is much more better (P<0.01). Both groups can alleviate patients'pain and improve the arthral function after one week or four weeks. And in the acute period, the study group improved better than the control treatment. After four weeks, the arthral function is no difference between the two groups, but the pain of the study group is still lighter than the control group(P<0.01). The recurrence rate in study group were obviously lower than the control group. No matter after one week, four weeks or twelve weeks, the laboratory parameters of both groups came lower than the last time examination. And the UA of the study group was lower than the control group in the same time. (P<0.01). Conclusion'Qingreliangxuefang' has a great curative effect on gout arthritis. It can reduce times of gout acute attacking, and improve the patients'living standard. It can keep UA in a low level in the intermission and reduce the recurernce rate. It has no toxic and side-effect. It can provide a long and effective method for treating gout arthritis.
