Practical Research On Social Work Intervention In Health Management Of Rural Elderly People With Chronic Disease

In recent years,China’s population aging has shown an accelerated trend,and a series of problems such as large scale,deep degree and empty nesting of the elderly in rural areas are the shortcomings of the national strategy in actively coping with population aging and healthy aging,coupled with the relative lack of medical resources in rural areas,the single access to health education,and the outstanding demand for health management of the elderly with chronic diseases.The professional value of "helping others to help themselves" upheld by social work is highly consistent with the implementation of the concept of positive aging and healthy aging,which is an important guarantee measure for building a high-quality elderly service system in rural areas,and an important social force for the health management of the elderly with chronic diseases in rural areas.Health Management for the Elderly with Chronic Diseases in Three Villages of Guangzhou is a medical project of "1KM Bridge" of SQ Town Social Work Station,which aims to explore the practical mode and promotion value of social work professionals to intervene in the health management of the elderly with chronic diseases in rural areas.Through a questionnaire survey of 120 elderly people with chronic diseases in three rural communities in Guangzhou,and 12 elderly people with special needs and village committees of three villages,this paper analyzes the current situation and main problems of health management of the elderly with chronic diseases,namely,poor awareness of independent health management,lack of learning channels for health management knowledge,limited access to health management medical resources,and insufficient social support network for health management,etc.,and analyzes the needs on this basis,that is,enriching the forms of health management activities,increasing the learning channels of health management,etc.Provide health management medical resources,build a health management support network,etc.Through comprehensive assessment,social workers used community work methods to intervene,link medical resources for the elderly with chronic diseases in the three villages,provide effective health management and prevention methods,improve the existing problems in community health management,help the elderly in the community cultivate scientific prevention concepts,standardize disease prevention behaviors,strengthen neighborhood peer relationships,and strengthen learning effects,so as to improve their self-health management capabilities.On the basis of community work methods,targeted casework methods were used to reach 2 clients through active communication and referral with others,and based on cognitive behavior theory,we helped clients change their wrong cognition of health management and establish correct health preservation concepts.Based on the social support theory,it helped client two to build informal network support and stimulate the motivation of health management.The evaluation of the effectiveness of the practical process shows that theoretical guidance is feasible and the application of social work methods has achieved certain results.Based on theoretical discussion,case analysis and reflection,it is concluded that: first,there are diversified needs of the elderly with chronic diseases at the level of health management services;Second,the elderly with chronic diseases are affected by multiple systems in terms of health management;Third,there are cognitive and behavioral factors that impair health in the elderly with chronic diseases;Fourth,social work has unique advantages in intervening in the health management of the elderly with chronic diseases.Based on this,the researchers made suggestions from the micro,meso and macro levels,aiming to improve the health management ability of the elderly with chronic diseases in rural areas.
