Objective: To investigate the clinical efficacy of Chaiping Tang with additional ingredients in treating functional constipation of liver depression and qi stagnation type.Methods: From January 2021 to December 2022,60 patients with liver depression and qi stagnation type functional constipation who visited the Colorectal Department of Inner Mongolia Autonomous Region Hospital of Traditional Chinese Medicine and met the inclusion criteria were selected for observation.All patients included in the trial were randomly assigned to either the observation group or the control group,with 30 patients in each group.The observation group was treated with Chaiping Tang with additional ingredients orally for 4 consecutive weeks,while the control group was treated with lactulose orally for 4 consecutive weeks.The total score of TCM syndromes and individual scores,the severity of constipation assessed by the Constipation Scoring System(CSS),the quality of life assessed by the Patient Assessment of Constipation Quality of Life(PAC-QOL)scale,and anxiety/depression assessed by the Depression and Anxiety Scale(DHA)were recorded before and after treatment.Statistical methods were used to compare the clinical efficacy between the two groups.Patients were followed up 2 months after treatment to calculate the recurrence rate and evaluate the prognosis of the two groups.Results:1、The total effective rate of TCM syndromes in the Chaiping Tang with additional ingredients group was 83.3%,while that in the lactulose group was 70.0%.The difference was statistically significant(P=0.021),indicating that the efficacy of Chaiping Tang with additional ingredients in improving TCM syndrome of constipation was superior to lactulose.2、After treatment,the total score of TCM syndromes and individual scores in both groups decreased compared to before treatment,with a statistically significant difference(P<0.001).The total score of TCM syndromes between the two groups after treatment was statistically significant(P<0.001).The differences were statistically significant in aggravation of bowel sound,fullness and oppression in the chest,frequent belching or hiccup,and emotional distress(P<0.05),but not statistically significant in bloating or abdominal pain and feeling of incomplete evacuation or difficulty in defecation(P>0.05).Therefore,the efficacy of Chaiping Tang with additional ingredients in improving bowel sound,fullness and oppression in the chest,frequent belching or hiccup,and emotional distress was superior to lactulose.3、After treatment,the Constipation Scoring System(CSS)score in both groups decreased compared to before treatment,with a statistically significant difference(P<0.001).The difference in CSS score between the two groups after treatment was statistically significant(P=0.002).Chaiping Tang with additional ingredients had a more significant effect in improving the severity of constipation than lactulose.4、After treatment,the total score of Patient Assessment of Constipation Quality of Life(PAC-QOL)in both groups decreased compared to before treatment,with a statistically significant difference(P<0.001).The difference in the total score of PAC-QOL between Chaiping Tang with additional ingredients and lactulose after treatment was statistically significant(P<0.001).Chaiping Tang with additional ingredients was superior to lactulose in improving the quality of life of constipation patients.5、After treatment,the Depression and Anxiety Scale(DHA)score in both groups decreased compared to before treatment,with a statistically significant difference(P<0.05).The difference in DHA score between the two groups after treatment was statistically significant(P<0.001).Chaiping Tang with additional ingredients was superior to lactulose in improving adverse emotions such as anxiety/depression in patients.6、After treatment,the Depression and Anxiety Scale(DHA)score in both groups decreased compared to before treatment,with a statistically significant difference(P<0.05).The difference in DHA score between the two groups after treatment was statistically significant(P<0.001).Chaiping Tang with additional ingredients was superior to lactulose in improving adverse emotions such as anxiety/depression in patients.7、All completed patients were followed up for 2 months after treatment,and their constipation symptoms were evaluated.The total recurrence rate of the control group was47.60%,while that of the observation group was 12.00%,indicating that the efficacy of the observation group was still superior to the control group after 2 months of follow-up.Conclusion: Both Chaiping Tang with additional ingredients and lactulose can improve the clinical symptoms of patients with functional constipation.The efficacy of Chaiping Tang with additional ingredients in improving TCM syndrome of constipation is superior to lactulose.It significantly improves the quality of life of constipation patients and adverse emotions such as anxiety/depression compared to lactulose,and its efficacy is worth affirming.
