Analysis Of The Current Situation Of Perinatal Birth Defects In An Urban Area Of Nanning City,2017-2020

Objective:To analyze the occurrence of perinatal birth defects and their influencing factors in an urban area of Nanning City from 2017 to 2020,to explore the characteristics of perinatal birth defects in this urban area in the past 4 years,the change pattern of trends and their influencing factors,and to provide reference for the subsequent development of comprehensive prevention and control measures.Methods:The data were obtained from the Information Management System of Gui Women’s and Children’s Health Services in Guangxi,from which the module of the list of perinatal birth defects in 21 medical institutions in an urban area of Nanning,Guangxi,from January 1,2017 to December 31,2020,was selected in accordance with the International Classification Criteria(ICD-10),the Implementation Plan for Birth Defect Monitoring in China,the Chinese Birth Defect Monitoring System for Difficult and Microscopic Malformations The report guidelines,the Outline of China’s Child Development Plan(2011-2020),the Guangxi Medical Intervention Program for Birth Defects(Trial),and the Guangxi Significant Mortality and Disability Birth Defects Evaluation Program were used to screen out 2993 children with 24 and other types of birth defects that met the diagnostic criteria for analysis.Results:(1)There were a total of 94,514 perinatal cases in an urban area of Nanning from 2017 to 2020,of which there were a total of 2993 cases of birth defects,and the incidence of birth defects was 31.67‰,and the incidence of birth defects in 2017,2018,2019 and 2020 were: 19.04‰,26.66‰,40.66‰and 45.87‰,showing a year-on-year increase Trend.(2)The main types of birth defects are: congenital heart disease 1195 cases(12.64‰),external ear and other malformations 349 cases(3.69‰),polydactyly298 cases(3.15‰),clubfoot 145 cases(1.53‰),and syndactyly 98 cases(1.04‰).Among the children with defects,there were 2825 single defects(94.39%)and 168 cases of two or more multiple defects(5.61%).(3)The higher the age of the mother,the higher the incidence of birth defects.The highest incidence of birth defects was 35.61‰ in the ≥35-year-old group,followed by 31.85 ‰ in the 30-year-old group,31.30 ‰ in the25-year-old group,26.78 ‰ in the 20-year-old group,and 25.23‰ in the<20-year-old group.(4)There were 50,801 perinatal cases of male infants and 1735 cases of birth defects,with an incidence rate of 35.71‰,and 43,708 perinatal cases of female infants and 1179 cases of birth defects,with an incidence rate of 26.97‰.The incidence of birth defects in male infants is higher than that in female infants.(5)The incidence of birth defects in 1 birth,2 births,3 births and above are37.61‰,27.81‰ and 27.87‰ respectively,with the highest incidence of birth defects in 1 birth.(6)the incidence of birth defects in preterm infants is 54.13 ‰,the incidence of birth defects in full-term infants is 29.86‰;the incidence of birth defects in birth length <50cm is 42.15‰,the incidence of birth defects in birth length ≥50cm is 27.22‰;the incidence of birth defects in low weight infants is 65.65‰,the incidence of birth defects in large infants is 43.72‰,normal weight infants The incidence of birth defects was 28.60 per thousand.(7)The proportion of postnatal diagnosis of birth defects in each year is above 85%,which is higher than the prenatal diagnosis rate;the total death rate of children with birth defects is 7.02 per 1,000.(8)There were 92,276 singleton perinatal births and 2,841 birth defects,with a birth defect incidence rate of 30.79‰;2,238 perinatal births of twins and above,with 152 birth defects,with a birth defect incidence rate of 67.92‰.(9)The results of binary multi-factor logistic regression analysis showed that the factors influencing birth defects were: maternal age,perinatal sex,number of births,and whether there were multiple births(P < 0.05).Conclusion:(1)The incidence of perinatal birth defects in an urban area of Nanning City remained at a high level from 2017 to 2020,and the incidence of perinatal birth defects showed an increasing trend year by year during the four years.(2)The main types of birth defects are congenital heart disease,other malformations of the external ear,polydactyly/toe,clubfoot,and syndactyly/toe.The proportion of birth defects diagnosed after birth is higher than that diagnosed before birth,indicating that the prenatal screening and diagnosis technology in the city needs to be improved.(3)Maternal age,perinatal sex,number of births,and whether there are multiple births may be related to the occurrence of birth defects,and the older the mother is,the greater the risk of perinatal birth defects.
