An Anylysis Of Perinatal Birth Defects Surveillance And Death Causes Among Children Under 5 Years Old In Futian District,Shenzhen City From 2009 To 2014

Objective: To analyze the characteristics and trends of perinatal birth defects and the characteristics and tendency of the mortality and the sequences of death causes under5 years old in Futian District, Shenzhen City from 2009 to 2014, And to provide the basis for the local maternal and child health care work.Methods: The information of perinatal birth defects and death in children under5-year-old between 2009 and 2014 was obtained from the maternal and child system of Shenzhen City. "Birth defects monitoring program in Guangdong province" was implemented to monitor the perinatal birth defects, and the method of monitoring was based on hospitals. “Death surveillance program for children under 5 years old in China” was executed to monitor the death cases of children below 5 years old. The data were analyzed by SPSS software after which were collected and sorted out.Results:1. It's the results of perinatal birth defects monitoring analysis from 2009 to 2014 in Futian District, Shenzhen City.(1) The occurrence rate of perinatal birth defects in Futian District is higher. The average occurrence rate of perinatal birth defects in Futian District from 2009 to 2014 is 27.72‰, which has been more than 20 per thousand each year and increased year by year.(2) In Futian district from 2009 to 2014, The incidence of male perinatal birth defects(27.59‰) was higher than that of female perinatal birth defects(25.25‰), but the mortality of female perinatal birth defects(4.63‰) is higher than that of male perinatal birth defects(4.00‰); The incidence(30.08‰)and the mortality(7.57‰) of birth defects of the floating population are higher than which of registered population(25.23‰, 2.52‰) and transient population(28.08‰, 4.60‰). The incidence(32.13‰, 27.71‰)and the mortality(6.28‰, 8.53‰) of birth defects of parturient woman with gravidity and parity were higher, but the incidence of birth defects of elder primipara(27.95‰) did not higher.(3) In Futian district from 2009 to 2014, perinatal birth defects mainly included congenital heart disease and body deformity. congenital heart disease is the primary one, followed by multi finger / toe, and external ear deformity(except for microtia and anotia), talipes equinovarus, hypospadias and so on.2. It's the analysis on the causes of death of children under 5 years old in Futian District, Shenzhen City from 2009 to 2014.(1) In Futian District from 2009 to 2014, the total mortality for children under five was 5.33‰, which was the highest in 2013(6.89‰) and more than the national average. The lowest mortality was in 2012(3.92‰).(2) In Futian District from 2009 to 2014, the infant mortality(72.16%) of children under 5 years old was the highest, and the neonatal mortality among infants was the highest(52.36%).(3) In Futian District from 2009 to 2014, the first three death causes of Newborns, infants and children under 5 years old were preterm birth and low birth weight(21.11%, 19.20%, 13.86%), congenital heart disease(16.26%, 13.77%, 9.93%)and other congenital defects(14.88%, 13.41%, 9.67%). The total proportion ratio between the five leading death causes was more than 50% each year. The death place of children under 5 years old was mainly in the hospital in Futian District(accounting for 80.13%).Conclusion:1. The total incidence of perinatal birth defects is higher and the birth defect is the main cause of perinatal death in Futian District, Shenzhen City from 2009 to2014.2. The incidence of birth defects and mortality in gender, residence, maternal age,gravidity and parity are different in Futian District, Shenzhen City from 2009 to 2014.3. Perinatal birth defects are mainly congenital heart disease, multi finger / toe,external ear deformity(except for microtia and anotia), talipes equinovarus,hypospadias in Futian District, Shenzhen City from 2009 to 2014.4. Compared with normal newborns, birth defects are more prone to premature birth and stunted growth and so on from 2009 to 2014.5. Under-five mortality is lower than the national level in Futian District,Shenzhen City.6. The infants are the most among the children died under 5 years old and the neonates are the most among infant died in Futian District, Shenzhen City.7. In Futian District, Shenzhen City, the first three main causes of death for children under the age of five are always preterm birth or low birth weight,congenital heart disease and other congenital defects, and the total proportion ratio between the five leading death causes was more than 50% each year.8. The prevention of birth defects can be integrated in the aspects of strengthening the perinatal health care, changing the unhealthy lifestyle and reducing the expose of risk factors because birth defects are influenced by many factors. And death prevention work for children under the age of five still needs to be strengthened,including strengthening the publicity of health care for pregnant women and children,improving medical level and the quality of obstetrics and pediatrics as well as implementing the maternal and child health work and so on.
