Pur Pose:Through the clinical retrospective analysis of the medical records of the patients with Chronic Urticaria in our hospital,the basic information of the patients,their symptoms and signs,laboratory examinations,and traditional Chinese medicine treatment were recorded in detail,the related factors of the disease and the characteristics of TCM treatment experience were analyzed.It is hoped that the TCM characteristic therapy of Chronic Urticaria can be summarized to Provide reference basis for clinical treatment based on syndrome differentiation.Methods:This study records the clinical data of patients diagnosed as chronic urticaria from November 2015 to November 2020,including General Information(name,sex,age,course of disease,actual length of hospital stay,etc.),season of onset,cause and inducement,Past disease history,allergy history,laboratory examination,clinical symptoms and signs,TCM Syndrome types,tongue coating and pulse condition,TCM Syndrome differentiation and treatment,etc.,to analyze the Chronic Urticaria clinical related factors and traditional Chinese medicine treatment methods.Results:1.In this study,213 patients with Chronic Urticaria were enrolled,including 64 male patients(30.05%),149 female patients(69.95%),1:2.33 male-female ratio,the age of patients with Chronic Urticaria ranged from 15 years old to 87 years old,with an average of 53.66±13.64 years old.The average length of stay was 11.72 ± 2.85 days.In spring(16.43%),in summer(32.39%),in autumn(25.82%)and in winter(25.35%),it was more common in summer.2.Clinical Symptoms And Signs: the first five skin manifestations were pruritus(100%),air mass(98.59%),scratch(16.90%),positive skin scratch sign(96.71%)and swelling(15.02%);the first five non-skin manifestations included dyspnea(25.82%),chest tightness(19.25%),palpitation(7.51%),difficulty in breathing(6.10%),feeling of holding breath(5.63%).3.Laboratory Examination: IgE,c-reactive protein can effectively help clinical monitoring of chronic urticaria and treatment.Allergen detection could identify the Allergen,the top 5allergens in food group were egg white,egg yolk(19.90%),wheat(19.90%),soybean(17.35%),peanut(16.84%),hazelnut/pistachio(13.27%),the top 5 allergens in inhalation group were house dust mite/dust mite(29.08%),penicillium SPP/aspergillus fumigatus/alternaria SP(21.94%),dog dander(19.90%),elm(19.39%).4.TCM Syndrome Types: 213 cases of apoplectic heat syndrome(85.45%),wind-cold syndrome(7.04%),blood deficiency and wind-dryness syndrome(6.1%),gastrointestinal damp-heat syndrome(0.94%),lung-wei-not-solid syndrome(0.47%).Chi-square test showed that there were significant differences in the distribution of syndrome types of chronic urticaria,and there were significant differences among different syndrome types in different course groups The distribution of syndrome types was independent of sex and age.5.Western medicine treatment: mainly antihistamines: 95 cases(4.60%)of(R)-CETIRIZINE,145 cases(68.08%)of EBASTINE,40 cases(18.78%)of AULOS,55 cases(25.82%)of chlorpheniramine,42 cases(19.72%)of loratadine and 4 cases(1.88%)of fexofenadine.The use frequency of compound glycyrrhizin injection was 83.57%,and that of vitamin C injection was 92.02%.6.Traditional Chinese Medicine Prescriptions: This study collected 178 prescriptions,involving 148 herbs,the total frequency of use of 2564 times.All the Chinese herbs were analyzed: the Top 5 were heat-clearing type(31.72%),deficiency-tonifying type(30.56%),antipyretic type(23.27%),diuresis and dampness type(4.25%),and liver-calming type(3.14%)The first five drugs were spleen Meridians(20.35%),liver meridians(18.08%),lung meridians(17.41%),heart meridians(13.91%),stomach meridians(10.08%),and the drugs were Gan(38.23%),ku(31.16%),xin(24.63%),acid(5.32%)and salty(0.67%)The drug properties were temperature(46.88%),cold(34.56%),flat(15.12%),cool(2.99%)and hot(0.44%).The Association analysis of 34 traditional Chinese medicines which were used more than 10 times was carried out and 46 association rules were obtained.Cluster analysis could cluster 3 kinds.Conclusion:1.In this study,the incidence of Chronic Urticaria was significantly higher in women than in men.The disease is susceptible to the ages from 40 to 59 years old,the disease course is 6 weeks to 1 year,attack season is summer.2.In this study,the detection of IgE and allergen can effectively help the clinical treatment of Chronic Urticaria.3.In this study,the TCM syndrome of Chronic Urticaria was mainly wind-heat syndrome,and the distribution of syndrome was related to the course of disease.4.In this study,antihistamines were mainly used in the treatment of chronic Urticaria,the application rate of Compound Glycyrrhizin and vitamin C was more than 80%.5.In this study,traditional chinese medicines commonly used in Chronic Urticaria were radix sa Poshnikoviae,radix glycyrrhizae,Radix Astragali,Radix Rehmanniae,radix angelicae sinensis,cortex moutan,cortex dictamni,Radix scutellariae,Rhizoma Atractylodis,Herba Schizonetae,Flos Lonicerae,etc..Common drug combinations are Radix Saposhnikoviae,radix astragali Radix et rhizoma glycyrrhizae Radix saposhnikoviae,cortex moutan radix rehmanniae Radix et Rhizoma saposhnikoviae.Tonifying and reducing simultaneously,regulating the balance of Qi,Blood,Yin and Yang,supporting the healthy and expelling evil,Promoting the recovery of diseases.
