Purpose:By collecting medical records of hospitalized patients with wind-fever phenotype urticaria, the pathogenesis factors, laboratory examination and the treatment of integrated traditional Chinese and western medicine were discussed,so as to explore new prescriptions and improve the diagnosis and treatment scheme,so as to provide reference for prevention and treatment in the future.Material and method:The medical records of patients diagnosed with wind-fever phenotype urticaria in the dermatology ward of Liaoning Provincial Hospital of Traditional Chinese Medicine from January 2017 to December 2020 were summarized,The general data(name, gender, age, length of stay, length of disease course, etc.),season of onset, accompanying symptoms, inducing factors, skin lesions, laboratory examination, and treatment course of traditional Chinese and western medicine were collated,The data were imported into Excel and tabulated, and analyzed by SPSS22.0 statistical software.Results:1.In this study, there were 158 patients, including 48 males and 110 females, ranging from 7years old to 87 years old.The mean age was 53.49±13.81 years old;The shortest length of hospitalization was 7d,the longest was 18 d,and the average length of hospitalization was12.42±2.58d;Seasons of onset: spring(20.89%),summer(33.54%),autumn(22.15%),winter(23.42%).2.Causing factors: 79.75% of the patients had no obvious inducement to develop the disease, and a few patients were induced by wind or improper diet.3.Disease history: 95 patients(60.13%)had a history of more than one or two diseases, including coronary heart disease, hypertension and diabetes mellitus.4.Allergy history: 52 patients(32.91%)had allergic history of one or more drugs, most of them were allergic to antibiotics.5.Concomitant symptoms: In addition to pruritus, the top 5 clinical symptoms were dyspnea(27.22%),local swelling(20.89%),chest tightness(14.56%),palpitation(6.33%),and shortness of breath(5.06%).6.Laboratory examination:More than half of the patients showed abnormal metabolic symptoms such as elevated blood glucose, lipid and uric acid, and the total serum Ig E in most patients was significantly elevated;Top 5 food allergies: egg whites, wheat, soy, milk, cod; The top 5 allergens in inhalation group were house dust mite and dust mite, dog dander, mold, cat dander and ragweed.7.Western medicine treatment:Compound glycyrrhizin injection and vitamin C injection were used as intravenous points,Forty-one patients received one antihistamine,104 patients received two antihistamines, and 13 patients received three antihistamines,The top 5 antihistamines in frequency of use were ebastidine tablets, levetirizine hydrochloride tablets, chlorpheniramine maleate tablets, olloratadine hydrochloride tablets and loratadine tablets,For patients with serious disease, timely oral administration of total glycosides of paeoniae paeoniae,Tripterygium wilfordii glycosides tablets,or intravenous or oral glucocorticoid to relieve symptoms.8.TCM(traditional Chinese medicine)therapy:According to the use frequency of oral decoction, the top 5 medicines in terms of efficacy were heat-clearing drugs(34.87%),deficiency tonifying drugs(26.88%),surface relieving drugs(23.39%),water-nourishing and humidifying drugs(3.92%),and liver-regulating and wind-extinguishing drugs(3.44%).The medicinal properties were cold(46.73%),warm(38.26%),flat(11.86%),cool(2.95%)and hot(0.19%).The medicinal taste was sweet(36.85%),bitter(30.68%),pungent(27.83%),light(2.28%)and sour(1.11%)in order.9.Curative effect evaluation:37 cases were cured.Significant effect in 106 cases;15 cases were improved; The efficiency was 90.50% in 0 casesConclusion:1.In this study, both men and women were affected by the disease. The most common age was between 51 and 70 years old,and the incidence was higher in summer.2.In this study, the oral effects of traditional Chinese medicine were mainly heat-clearing drugs and deficiency tonic drugs.3.In this study, the top ten drug use frequency of the phenotype of stroke fever urticaria were windbreak(6.73%)>licorice(6.54%)>herba schizonepetae(6.54%)>atractylodes(6.34%)>radix rehmanniae(6.15%)>cortex dictamni(5.81%)>cortex moutan(5.81%)>angelica(5.67%)>astragalus membranaceus(5.47%)>scutellaria baicalensis(5.47%).4.It is concluded that C1,a potential new formulation, can treat chronic urticaria with spleen deficiency and dampness.C2 is suitable for patients with wind-heat syndrome or chronic urticaria with insufficiency of qi and blood,C3 can be used as a basic prescription for patients with chronic urticaria with wind evil and heat evil invading muscle surface or with Qi and Yin deficiency.5.The study showed that the effective rate of integrated traditional Chinese and western medicine in the treatment of this disease was 90.50%,and the effect was significant.
