| Objective: Mixed hemorrhoids are a benign anorectal disease with high incidence rate.Surgical treatment is the most common method in clinic,while urinary retention is the most common postoperative complication.In this study,Qingliyiqi Powder was used to treat patients with postoperative retention of urine after hygrothermal mixed hemorrhoids.The aim was to find a therapeutic regimen with little side effect,improve the quality of life and create social benefits,and provide data support for clinical application of Qingliyiqi Powder.Methods: In this study,the patients with mixed hemorrhoid caused by downward flow for damp-heat who received the injection therapy of internal hemorrhoids and Milligan-Morgan hemorrhoidectomy from January 2020 to December 2020 were selected as the observation objects,according to the principle of minimum distribution of imbalanced index,according to the order of postoperative urinary retention,66 patients who met the criteria were randomly divided into treatment group and control group,33 cases in each group.In order to ensure the balance of the trial,all the patients were treated with the same operation method,anesthesia method and perioperative management plan.After the treatment is ineffective through psychological induction and environmental induction,the treatment group was treated with half dose of Qingliyiqi Powder(all with50 ml warm water),half dose again after 1 hour.The symptoms of the patients were observed after 2 hours.If the symptoms could not be relieved or aggravated,it was judged as invalid and catheterization was given;if the symptoms were relieved,the patients continued to take it from the 2nd to 10 th day,half dose in the morning and half in the evening every day,and the total course of treatment was 10 days.The control group was treated with Longbishu Capsule 0.9g(both with 50 ml warm water),and took 0.9g again one hour later.The symptoms of the patients were observed after taking the medicine for 2hours.If the symptoms could not be relieved or aggravated,it was judged as invalid and catheterization was given;if the symptoms were relieved,they were continued to take 0.9g in the morning and evening every day for 10 days.The general information(age,gender,course of disease,degree of internal hemorrhoids,number of hemorrhoids,operation time)of the patients were observed;the first postoperative micturition time and volume;the first postoperative micturition time and volume after the treatment;urination score and curative effect judgment of patients before treatment and on the 3rd and 10 th day of treatment;the safety index and adverse reactions were observed.Results: There was no significant difference in the first postoperative micturition time and volume(P=0.430,P=0.555)(P>0.05),which was comparable.The two groups of patients in the first micturition time after treatment,the difference was statistically significant(P<0.01),the treatment group was better than the control group;in the first micturition volume after treatment,the difference was statistically significant(P=0.034<0.05),the treatment group was better than the control group.Two significant(P=0.029<0.05);the comparison of bladder percussion scores,Z=-1.703,the difference was not statistically significant(P=0.088>0.05).Two groups of patients in the treatment of the 10 th day of urination each factor score comparison: the urination score comparison,Z=-2.454,the difference was statistically significant(P=0.014<0.05);the comparison of abdominal symptom scores,Z=-2.653,the difference was statistically significant(P=0.008<0.01);the comparison of urination time scores,Z=-2.088,the difference was statistically significant(P=0.037<0.05);the comparison of bladder percussion scores,Z=-2.448,the difference was statistically significant(P=0.014<0.05).Two groups of patients before treatment and in the treatment of the 3th day and the10 th day of urination total score comparison: two groups before treatment total score comparison Z=-0.138,the difference was not statistically significant(P=0.890>0.05),before treatment was comparable;two groups on the 3th day total score comparison Z=-2.208,the difference was statistically significant(P=0.027<0.05),the treatment group was better than the control group;two groups on 10 th day total score comparison Z=-2.859,the difference was statistically significant(P=0.004<0.01),the treatment group was better than the control group.Clinical curative effect: the short-term curative effect comparison of the two groups after 3 days of treatment: in the treatment group,7 cases were cured,9 cases were markedly effective,13 cases were effective,4 cases were invalid,the cure rate was 21.21%;in the control group,3 cases were cured,5 cases were markedly effective,18 cases were effective,7 cases were invalid,the cure rate was 9.09%;the difference between the two groups was statistically significant(P=0.047<0.05).Comparison of 10 day long-term efficacy between the two groups: in the treatment group,18 cases were cured,9 cases were markedly effective,2 cases were effective,4 cases were invalid,the cure rate was 54.54%;in the control group,10 cases were cured,8 cases were markedly effective,7 cases were effective,8 cases were invalid,the cure rate was 30.30%;there was significant difference between the two groups(P=0.022<0.05).Conclusion: Qingliyiqi Powder is better than Longbishu Capsule in treating retention of urine after the injection therapy of internal hemorrhoids and Milligan-Morgan hemorrhoidectomy of mixed hemorrhoid caused by downward flow for damp-heat.It can effectively shorten the urination time of patients with postoperative urinary retention,increase the voiding volume of patients with urinary retention,and improve the voiding condition of patients with postoperative urinary retention.Qingliyiqi Powder has the effects of reinforcing Qi and enriching blood,clearing heat and dampness,operating Qi and relieving pain.It is a safe,simple and effective treatment method,which is worthy of clinical application. |
