Objective Helicobacter pylori(Hp)is one of the few microorganisms that can adapt to the harsh environment of stomach.It is closely related to a variety of digestive diseases,and the worldwide infection rate of Hp has been more than 50%of non-alcoholic fatty liver disease(Nonalcoholic fatty liver)NAFLD is a metabolic and genetic liver injury that has been recognized as a global public health problem.NAFLD affects 20%to 45%of the general population and its prevalence is on the rise worldwide.The progression of NAFLD can lead to cirrhosis and hepatocellular carcinoma It was found that Hp infection was one of the causes of NAFLD,and eradication of Hp could delay the progression of NAFLD to a certain extent.At present,the relationship between Hp and NAFLD is still controversial,and the conclusion is still unclear.The purpose of this study is to explore the relationship between Hp infection and NAFLD,so as to provide reference for clinical treatment of NAFLD.Method Subjects who underwent 14C-urea breath test and abdominal Ultrasonography during June 2019 to June 2021 in Hefei First People’s Hospital were selected to collect and record their general information(gender,age,height,weight,blood pressure,smoking history,drinking history,drug use history,history of hypertension,history of cardiovascular and cerebrovascular diseases)History of diabetes,etc.),blood routine blood glucose,liver function,blood lipid,kidney function and other blood chemical test results,abdominal B-ultrasound results,14C-urea breath test results were determined according to the inclusion and exclusion criteria to analyze the NAFLD detection rate and Hp positive rate in the study population,as well as the trend of their changes with age According to the results of abdominal Ultrasound,the study population was divided into NAFLD group and control group.The differences of general data,blood biochemical indexes and Hp positive rate between the two groups were compared.The NAFLD group was further divided into mild,moderate and severe fatty liver according to the degree of steatosis,and the difference of Hp positive rate among the three groups was compared According to the results of 14C-urea breath test,the study population was divided into the Hp infection group and the non-HP infection group,and the differences of general blood biochemical indexes and NAFLD detection rate between the two groups were compared.Finally,the risk factors of NAFLD were determined by multi-factor regression analysis.Results(1)A total of 393 cases(40.9%)of NAFLD were detected in 960 subjects,and 412 cases(42.9%)were Hp positive.The positive rates of Hp and NAFLD were highest in people aged 50-59 years(44.5%and 49.8%,respectively)The hP-positive and NAFLD detection rates generally increased with the increase of age.(2)After grouping subjects according to BMI,the positive rate of HP-positive and NAFLD detection rates in obese and overweight subjects were significantly higher than those in normal and emaciated subjects.(3)The analysis results of general data of NAFLD group and control group suggested the average age BMI value of NAFLD group Smoking history history of hypertension history of systolic blood pressure diastolic blood pressure history of diabetes was significantly higher than that of the control group,while there was no significant difference between the two groups of gender.(4)Blood biochemical index analysis results suggested that compared with the control group,absolute value of WBC neutrophils PLT ALT AST GGT ALP TG TC LDL-C uric acid was found in NAFLD group The levels of FBG,HbAlc and HDL-C were higher,while the levels of albumin,urea,nitrogen and creatinine were not significantly different.(5)The positive rate of Hp in NAFLD group and control group was 49.36%38.45%,respectively.After grouping according to blood lipid level,the results showed that the Hp positive rate of NAFLD patients with dyslipidemia was significantly higher than that of those without dyslipidemia.After classifying NAFLD patients into mild,moderate and severe fatty liver according to b-ultrasound description,there was no significant difference in Hp infection rate.(6)According to the group of Hp infection,the analysis of general data between the two groups showed that the average age of the Hp positive group,BMI,smoking history,history of hypertension,history of diastolic blood pressure,and history of diabetes were significantly higher than those of the control group,while there was no significant difference in gender systolic blood pressure between the two groups.(7)Absolute value of WBC neutrophils in the Hp positive group,AST,GGT,ALP TG TC LDL-C level was higher than that of the HP-negative group,while HDL-C level was lower than that of the HP-negative group.The difference was statistically significant.The prevalence of NAFLD in the HP-positive group was 47.82%,which was significantly higher than that in the HP-negative group(35.77%).(8)TG HbA1c Hp infection is a related factor of NAFLDConclusion(1)Obesity,history of smoking,history of hypertension,diabetes,and higher risk of NAFLD in the elderly.(2)Evidence of metabolic disorders in people with Hp infection.(3)Age,BMI,TG,HbA1c,Hp infection is a risk factor for NAFLD.(4)Hp is involved in the development of NAFLD.
