Clinical Observation On The Treatment Of Handgrip After Stroke With Penetrating Method

Objective: To observe the clinical effect of penetrating puncture for the treatment of hand-holding after stroke,to find a simpler and safer treatment method,to alleviate the pain of patients and improve the quality of life.Methods: The sample of this study was selected from February 2019 to December 2019 in patients who were hospitalized and treated in the acupuncture department of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine,and selected to be consistent with cerebral infarction or cerebral hemorrhage diagnosis and stroke A total of 64 cases with diagnostic criteria for posterior grasping were randomly divided into 1: 1 groups according to the random number table method,and were divided into an observation group of 32 and a control group of 32.One patient fell off in each group.All patients are given basic treatment according to the actual situation.Patients with cerebral infarction are given conventional treatments such as anti-platelet aggregation,anticoagulation,stabilization of plaque,improvement of cerebral circulation,and improvement of cerebral metabolism;patients with cerebral hemorrhage are given basic treatments such as dehydration,intracranial pressure reduction,and nutritional nerves.;Both groups should control blood pressure,control blood sugar,control blood lipids and so on.Regularly check the patient's blood,urine,stool routine,coagulation function,D-D dimer,biochemical items and other indicators,pay close attention to changes in vital signs,changes in electrocardiogram,etc.In terms of acupuncture treatment,both groups of patients were given conventional acupuncture treatment.According to Shi's "Xingnao Kaiqiao Method" established by Shi Xuemin,acupoint selection included Neiguan,Shuigou,Sanyinjiao,Jiquan,Weizhong,Chize,etc.Governance is based on awakening the brain and inducing enthusiasm.On the basis of conventional acupuncture,the observation group was given acupuncture treatment through the acupuncture method,namely Heguto Sanjian,Hegutouhouxi,and Houxitouhegu.The control group was given traditional acupuncture at Baxie.Acupuncture treatment was given once a day for 6 days,acupuncture was stopped for 1 day,and the course of treatment was 28 days.By observing and analyzing the changes in the three scales of patients before and after treatment: Modified Ashworth Spasm Assessment Standard(MAS),Modified Barthel Index Scoring Standard(BI),and Extremity Limb Motor Function Measurement(FMA),we explored the use of transpuncture method to treat stroke The clinical effectiveness of holding the back of the hand,and to explore the safety and compliance of the treatment by analyzing adverse events and patient compliance.Results: The two groups of samples were not statistically significant in comparison of basic information such as gender,age,etiology,and course of disease(P> 0.05),and were comparable.At the same time,the efficacy evaluation results of the two groups of patients before treatment were comparable(P> 0.05).After 14 days of treatment,the clinical efficacy of the two groups was not obvious(P> 0.05).Most patients had MAS,FMA,and BI.The score did not change significantly or even worsened,and the symptoms of a small number of patients improved slightly.After 28 days of treatment,the MAS score(Z =-4.585),FMA score(t =-11.158),BI score(Z =-4.889)of the observation group were improved compared with before treatment(P <0.05),and the MAS score of the control group(Z =-3.247),FMA score(t =-5.884),BI score(Z =-5.324)were also better than before treatment(P <0.05),suggesting that the two groups of therapies improved hand grip after improving stroke All have therapeutic effects.At the same time,the total clinical effectiveness of the observation group has a clear advantage compared with the control group(Z =-2.341,P = 0.019 <0.05).Conclusion: Both traditional Xingnao Kaiqiao acupuncture treatment and penetrating acupuncture can effectively reduce the level of hand spasm in patients with sturdy hands after stroke,improve hand motor function and enhance their self-care ability.At the same time,on the basis of Xingnao Kaiqiao,The acupuncture method has more advantages than the traditional acupuncture method in the treatment of stroke patients with hand holding.
