Reflex sympathetic dystrophy after stroke, also called shoulder-hand syndrome (SHS),is a common complication of stroke. It is presumed that SHS is consequence of many factors, including periphery injury, effect of sympathetic never system, neurogenic inflammation, and plasticity of central nervous system. SHS often happens in1to3month after stroke., most commonly affects a limb. And only about20%patients would have recovered fully, without moderate treatment, the shoulders and hands’ function will be lost and deformity forever. According to the clinical characters and profound degree, SHS is divided into three grades, in this study, we designed to investigate the clinical effects of penetration acupuncture for Ⅰ period of SHS patients. ObjectiveIn this study, we selected the six groups of acupionts which originated by master Wang Yue-ting, and employed the method of penetration acupuncture for SHS patients. And contrast with the common acupuncture method, through comparison of VAS and Fugl-Meyer scores of60patients, before and after treatment. We observe its clinical effect and analyzed the mechanism. Methods60patients were randomly divided into treatment group and control group. There were30patients each group. Patients in the treatment group which treated with penetration acupuncture, and control group which treated by commom acupuncture method on the basis of textbook, the patients accepted treatment one times a day, consecutive15times as a course, the efficacy of the both group were be compared after2course of treatment. The observe indexes were the Visual Analog Scale(VAS) and the Fugl-Meyer scale. We compared the scores of VAS and Fugl-Meyer before and after the treatment, finally used the SPSS software to statistical data analysis and conculsions. Results 1. Before the treatment, the difference of VAS score between the two groups was not significant(P>0.05), the data had comparable. After adopted each acupuncture method, the VAS score was decreased in both group, and the downward trend in observed group was more remarkable. and there was significant difference between the two groups (P<0.01), which indicated that penetration acupuncture for SHS patients had better pain-lessening effect.2. According to Fugl-Meyer scale, There was no significant difference between the two groups Before the treatment (P>0.05). after2course of treatment, the Fugl-Meyer scale scores improved obviously, and the observed group had better superiority, the difference was significant(P<0.05). The penetration acupuncture treating advantages in ameliorating the coordinating movement and the active function of upper limb.3. According to the clinical efficacy of the long-term effect, in treatment group, the clinical recovery was7, markedly effective16cases, effective5cases, ineffectiveness1cases, the effective rate was93.3%, while in control group, the clinical recovery was4, markedly effective11cases, effective8cases, ineffectiveness7cases, the effective rate was76.7%, The penetration acupuncture treating has better clinical effect in cliical trial than common acupuncture treatment.(P<0.05)ConclusionThe results demonstrated that the method of penetration acupuncture for SHS patients, could remarkably alleviated the pain, ameliorate the coordinating movement of upper limb, and the active function of shoulder, elbow, wrist, finger and omit. The treating advantages in acupionts simplified, stimulation strongly, afficacy well, and is worth applying and spreading.
