Objectives:1.To investigate the allocation and utilization of pre-hospital first aid resources in the main urban area of Zunyi;2.To analyze the problems existing in the allocation and utilization of pre-hospital first aid resources in the main urban area of Zunyi;3.To provide theoretical basis and reference for further optimizing the allocation and utilization of emergency resources in the main urban area of Zunyi CityMethods:1.The Zunyi City pre-hospital first aid resources status questionnaire was used to collect the official raw data(archive data or electronic data)of the first aid agencies in Zunyi City main urban area,including general information of first aid agencies,first aid vehicles,first aid equipment and other first aid resources.2.Use the statistics table of emergency outpatient data to collect pre-hospital emergency outpatient data from 2017 to 2019 from the dispatching system of the emergency center of Zunyi City and first aid substations,including basic information such as the time,place,preliminary diagnosis,gender,and age of the outpatient.3.Use the general information survey form for first aid personnel to collect sociodemographic characteristics and basic work conditions such as name,gender,education,job title,working life,employment methods,etc.4.Using a convenient sampling method,select some experts in the field of pre-hospital emergency treatment for interviews.Understand the current situation of the allocation of pre-hospital emergency resources in Zunyi City,the existing deficiencies,and explore corresponding countermeasures.Through statistical analysis,understand the allocation and utilization of pre-hospital emergency resources in Zunyi CityResults:1.There were a total of 14 first aid substations in Zunyi City main ur ban area.2.Pre-hospital emergency resource allocation:?Ambulance resources:a total of 57 pre-hospital emergency vehicles.?Human resources:less than 40 yea rs of age,undergraduate education,below junior professional title,working life o f less than 10 years and other variables account for a large proportion;There is no statistically significant difference in gender between factors such as establishm ent,age,education,job title,and number of years of work(x2=0.430,P=0.512;?2=0.593,P=0.898;?2=3.104,P=0.212;?2=3.634,P=0.304;?2=0.357,P=0.873)?First aid equipment:The distribution of portable ventilator,cardiac defibrillator,ecg monitor,endotracheal intubation device,sputum aspirator and oxygen cylind ers in the four districts was statistically significant(?2=9.547,P=0.023;?2=7.911,P=0.048;?2=7.911,P=0.048;?2=7.911,P=0.048;?2=7.911,P=0.048;?2=9.391,P=0.025;?2=15.528,P=0.001),The proportion of the above equipment is the hig hest in Huichuan District.3.Analysis of disease spectrum of emergency patients:?The peak months of visits are July,August,and May.?The order of daily vi siting peak time is 10:00?12:00(11.43%),08:00?10:00(10.21%),12:00?14:00(9.83%);The lowest three places in the order of troughs every morning are 06:00?08:00(5.23%),02:00?04:00(5.14%),and 04:00?06:00(3.84%).?The first six p atients with pre-hospital emergency treatment were:18052 cases of trauma(30.99%),13805 cases of neurological disease(23.70%),7797 cases of cardiovascular disease(13.38%),and 3482 cases of obstetrics and gynecology(5.98%),3311 ca ses of digestive system diseases(5.23%),1779 cases of endocrine system disease s(3.05%).?The composition ratios of different factors(Sex,age,nationality,edu cation,region,quarter,marital status,time of visiting)in different years are diffe rent(?2=12.557,P=0.02;?2=91.885,P<0.001;?2=36.55,P<0.001;?2=32.797,P<0.001;?2=421.445,P<0.01,?2=44.578,P<0.001;?2=31.885,P<0.001;?2=55.097,P<0.001).4.Fairness analysis:The Gini coefficients of ambulance resources and human resources according to population of Zunyi City main urban area are 0.1502 and 0.1332,the Gini coefficients of ambulance resources according to the geo graphical area of Zunyi City main urban area are 0.1713,and the Gini coefficien t of emergency human resources according to the geographical area The coefficie nt is 0.2663,which indicates that the above resource allocation is in a fair state.Conclusion:(1)Optimization of pre-hospital first aid resource allocation in the main urban area of Zunyi has made some progress,but there are also some pr oblems.The uneven distribution of emergency stations in the main urban area of Zunyi leads to the corresponding extension of the average response time of am bulances.(2)There are some deficiencies in the number of first aid vehicles,on-board first aid equipment and first aid human resources in the main urban area of Zunyi city.Provincial,municipal and district governments have increased finan cial input,increased the number of emergency vehicles,and made up for the on-board emergency equipment;We will strengthen supervision over the implementat ion of relevant projects and funds,effectively ensure that first-aid material resour ces are replenished,improve the treatment of first aid workers,set up post allow ances,and take multiple measures to stabilize the ranks of first aid personnel.(3)The first aid human resources and ambulance vehicle resources in the main urb an area of Zunyi city are fairly allocated according to population and geographic al area.(4)According to the disease spectrum of first-aid patients in the main u rban area of Zunyi City,and according to the changing characteristics of first-aid needs in different time periods,seasons,regions,etc.,rationally allocate first-aid resources.According to the implementation of flexible scheduling for first aid pe rsonnel,targeted training of first aid knowledge and skills.
