Objective:Understand and analyze the current status of nurse organization silent behavior,job involvement and job satisfaction and its main influencing factors,explore the relationship between the organizational silence of nurse,job involvement and job satisfaction,and reduce the nurse organizational silence for clinical nursing managers and improve Provide a theoretical basis for nurses' job input level and increase job satisfaction.Methods:From March to April 2019,437 clinical nurses from three tertiary first-class general hospitals in Handan City,Hebei Province were selected by the convenient sampling method as the research object.A cross-sectional study was used to collect data through a self-designed demographic basic characteristics questionnaire,organizational silence scale,Utrecht job input scale,and nurse job satisfaction scale(MMSS).Pearson ?~2 test and multi-factor logistic regression were used to study the relationship between the basic characteristics of the nurse population and job input,organizational silence,and job satisfaction;and the work was studied by Spearman correlation analysis,multiple linear regression analysis,and structural equation models of the relationship between the organizational silence of nurse,job involvement and job satisfaction.Results:1.The results showed that the overall work input of 437 clinical nurses was divided into(3.44 ± 1.30)points,and the scores from high to low in the three dimensions were vitality(3.53 ± 1.41)points,concentration(3.39 ± 1.30)points,and dedication(3.38 ± 1.45)points.Organizational silence is generally divided into(2.67 ± 0.68)points,and the three dimensions are ranked in descending order of tacit silence(3.18 ± 0.92)points,defensive silence(2.83 ± 0.87)points,and indifferent silence(2.02 ± 0.74)points.The overall job satisfaction is divided into(3.11 ± 0.57)points,and the scores from high to low in the eight dimensions are as follows: satisfaction with colleagues(3.69 ± 0.64)points,praise and recognition(3.36 ± 0.65)points,control And responsibility(3.30 ± 0.63)points,social opportunities(3.14 ± 0.71)points,professional development(3.12 ± 0.71)points,shift scheduling(2.90 ± 0.74)points,welfare benefits(2.66 ± 0.84)points,family and work balance(2.57 ± 0.90)points.2.Educational background,marital status,and the total scores of nurses' work input in their departments are statistically significant(P<0.01);marital status is statistically significant in the total score of silent organization of the nurses(P < 0.01);labor and personnel relations,where There was a statistically significant difference in total job satisfaction scores among nurses in the department(P<0.01).3.Negative correlation between nurse organizational silence and job involvement(r=-0.254;P < 0.01);Negative correlation between nurse organizational silence and job satisfaction(r=-0.345;P < 0.01);job involvement and job satisfaction Degrees were positively correlated(r=0.560;P<0.01).4.The model fits well,the mediating effect of work input between organizational silence and job satisfaction exists,and organizational silence negatively affects job input(?=-0.30;P<0.001),and job input positively affects job satisfaction(?=0.52;P<0.001),the direct effect of organizational silence on job satisfaction is-0.220,the total effect is-0.376,the direct effect accounts for 58.51% of the total effect,and the intermediate effect value is-0.156.Conclusion:1.The silent organization of nurses in the top three hospitals in Handan area is at a moderately high level,and there are negative opinions because they feel that their suggestions will not change the status quo.Although the nurse's work input is at a moderately high level,it is lower than in first-tier cities.Lower than in European and American countries,there is still much room for improvement.The job satisfaction of nurses is at a moderately low level,and job satisfaction is not high.2.Married nurses have more severe organizational silence than unmarried nurses.The job input level of nurses increases with the increase in education level;the job input level of unmarried nurses is higher than that of married nurses;the job input level of nurses in operating rooms is the highest,followed by outpatient and pediatrics.The nurses in the establishment have higher job satisfaction than the contract nurses;outpatient nurses have the highest job satisfaction,followed by obstetrics and gynecology,pediatrics,operating rooms,and surgery.3.The lower the level of nurse organization silence,the higher the job satisfaction.The higher the level of nurses' work involvement,the more satisfied they are with their work.The lower the level of nurse organizational silence,the worse the job engagement status.Job involvement plays a mediating role between organizational silence and job satisfaction,that is,nurse organizational silence can indirectly affect job satisfaction by affecting job engagement levels.Nurses with low levels of organizational silence and nurses with good work engagement status have higher job satisfaction.Nursing managers in medical institutions need to pay attention to the actual impact of organizational silence and job input on job satisfaction.According to the differentiated characteristics of nurses,adopt personalized countermeasures,and strive to reduce or control the organizational silent situation in nurses,thereby improving the work enthusiasm,improve job satisfaction of nurses,and bring more high-quality nursing work for patients.
