A Correlation Study On Nurse's Silence Behavior,nurse's Work Alienation And Organizational Climate In A Tertiary Hospital In Guangzhou

Objectives: 1.To investigate the silence behavior,work alienation and organizational climate in nurses.2.To analyze the correlation and the pathway among silence behavior,work alienation and organizational climate.Methods: 1.A cross-sectional design was used.The data were collected with a general information questionnaire,Silence Behavior Scale,Nurse's Work Alienation Scale and Nurses' Organizational Climate Scale.2.Convenience sampling was used to investigate 603 registered nurses from a tertiary hospital in Guangzhou during July 2014 to August 2014.3.Data were analyzed by SPSS18.0 and AMOS17.0.Statistical methods include descriptive analysis,Mann-Whitney U Rank Test,Kruskal-Wallis H Rank Test,Spearman correlation analysis,regression analysis and path analysis.Results: 1.Total average score of nurses' silence behavior was(3.13±0.54).The entries averages of each dimension were: acquiescence silence(3.55±0.68),defensive silence(3.14±0.63),disregard silence(2.72±0.73).Total average score of nurses' work alienation was(2.86±0.54).The entries averages of each dimension were: sense of inactivity(3.46±0.71),meaninglessness(2.95±0.62),helplessness(2.17±0.68).Total average score of nurses' organizational climate was(3.73±0.36).The entries averages of each dimension were: interpersonal climate behavior(3.88±0.51),support fair behavior(3.85±0.43),colleague behavior(3.76±0.44),intimate climate and aggressive behavior(3.32±0.53).2.The score differences of nurses' silence behavior on different ages,service years,educations,job titles,departments and salaries were statistically significant(P <0.01).The score differences of nurses' silence behavior on different sex,marital status and personnel relations were no statistically significant(P> 0.05).3.Silence behavior was positively related to work alienation(r=0.603)and each dimension was positively related to work alienation(P <0.01).Silence behavior was negatively related to organizational climate(r =-0.344)and each dimension was negatively related to organizational climate(P <0.01).Work alienation was negatively related to organizational climate(r =-0.421)and each dimension was negatively related to organizational climate(P<0.01).4.The model goodness of fit was well.Organizational climate had direct effect on silence behavior and the normalized value was-0.12.Work alienation had direct effect on silence behavior and the normalized value was 0.84.Work alienation played a mediating role between organizational climate and silence behavior,and the normalized value was-0.43.Conclusions: 1.In this research,nurses' silence behavior was in a medium level,and attitudes were reserved by nurses since they believed they were unable to change current status.Nurses' work alienation was in a medium level.Partial of them felt helpless for their work,and believed nursing work was meaningless.The organizational climate was above medium level,and nurses had good organizational climate feeling.2.Nurses with younger age and less service years had lower silence behavior level.Nurses with age between 26-40 years old and service years between 4-20 years had higher silence behavior level.Nurses with service years above 20 years had the lowest silence behavior level.The higher educational background nurses had,the lower silence behavior level they would have.Compared with nurses with title of nurse and lower salary,nurses with title of senior nurse had higher silence behavior level.For nurses with title higher than senior nurse,the higher title and salary they had,the lower silence behavior level they would have.Nurses in operating room had the highest silence behavior level,followed by nurses in internal medicine departments,obstetrics and gynecology departments and emergency departments.Surgical nurses had the lowest silence behavior level.3.The stronger nurses' work alienation was,the higher level of nurses' silence behavior was.The better organizational climate was,the lower level of nurses' silence behavior and work alienation were.Work alienation played a mediating role between organizational climate and silence behavior.Organizational climate can affect nurses' silence behavior by affecting nurses' work alienation.Nurses who had a low level of work alienation and in a good department will had a low level of silence behavior.
