Objective:To investigate the correlation between adiponectin and helicobacter pylori(Hp)infection with nonalcoholic fatty liver disease(NAFLD),and to analyze the influencing factors of NAFLD.Methods:Patients diagnosed with non-alcoholic fatty liver disease in the outpatient department of gastroenterology and physical examination center of Mawangdui campus of Hunan Provincial People's Hospital from May 2019 to January 2020 were selected as the case group(n =102),including 53 males and 49 females.A total of 53 males and 53 females were selected as the control group(n=106).Collecting each object data(age,gender,height,weight,body mass index(BMI),systolic pressure,diastolic blood pressure,waist circumference,hip circumference,etc.),and getting a color doppler ultrasound or CT scan of the abdomen,testing C14 breath and relating biochemical examination,routine blood,liver and kidney function,fasting blood sugar and blood fat),and collecting each object in the serum with ELISA method to measure the insulin and adiponectin,insulin resistance index evaluation method based on steady state model calculation.Analyzing the differences in general conditions,biochemical indicators and Hp infection rate between the two groups,and the indicators with statistical significance in the univariate analysis were analyzed by Logistic regression.According to the C14 breath test result,the case group and the control group were further divided into the Hp infection group and the non-hp infection group,and the general data and biochemical indicators between the two groups were analyzed.Data,using statistical softwareSPSS24.0 of non-normal distribution measurement data using M(P25,P75)said,with a comparison between the two groups of testing parameters,count data comparison between the two groups by chi-square test,Spearman correlation coefficient analysis is adopted to improve the serum adiponectin and the relationship between the detection index,using the binary Logistic regression analysis the influence factor of NAFLD.Results:1.Body weight,BMI,waist circumference,waist-to-hip ratio,systolic blood pressure and diastolic blood pressure in the NAFLD group were higher than the control group,with statistically significant differ ences(P<0.01).2.Uric acid,aspartate aminotransferase,alanine aminotransferase and glutaminyl transferase,fasting blood glucose,fasting insulin,insulin resistance index,total cholesterol,triglycerides,and LDL in the NAFLD group were higher than the control group,while adiponectin and HDL were lower than the control group,with statistically significant differ ences(P<0.05).3.The infection rate of Hp in patients with NAFLD was higher than that in the control group,with no statistically significant difference(P >0.05).4.In the NAFLD subgroup,diastolic blood pressure,blood glucose,insulin resistance index and uric acid in the hp-infected group were all higher than those in the non-hp-infected group,while high-density lipoprotein and adiponectin were lower than those in the non-hp-infected group,with statistically significant differences(P<0.05).In the control group,blood glucose,insulin resistance index and triglycerides in the Hp infection group were higher than those in the non-hp group,and adiponectin was lower than those in the control group,with statisticallysignificant differences(P<0.05).5.Spearman correlation analysis showed that the serum adiponectin and body weight,BMI,waist circumference,waist-hip ratio,systolic pressure,diastolic pressure,cereal third transaminase,aspertate amino transferase,GGT(gamma glutamyl transpeptidase),fasting plasma gluc ose,insulin,insulin resistance index,total cholesterol,triglyceride,low density lipoprotein cholesterol(hdl-c)showed a negative correlation,and high-density lipoprotein cholesterol(hdl-c)were positively correlated,P< 0.05).6.Systolic pressure,ALT,,insulin resistance index and triglycerides are risk factors for NAFLD,while HDL and serum adiponectin are protective factors for NAFLD.Helicobacter pylori infection is not a risk factor for NAFLD.Conclusion:1.Serum adiponectin reduction is an independent risk factor for NAFLD,and serum adiponectin is closely related to NAFLD and metabolic diseases.2.Risk factors for NAFLD include systolic blood pressure and metabolic disorders such as blood sugar and lipid,while Hp infection is not an independent risk factor for NAFLD.3.Hp-infected people have insulin resistance,and serum adipon ectin is low in hp-infected people.
