Objective:1.To verify the effectiveness and safety of cervical balance regulation and acupuncture on the basis of structure differentiation theory for the treatment of cervical vertigo;2.To explore the mechanism of action of cervical balance regulation and acupuncture for the treatment of cervical vertigo,and lay a theoretical foundation.3.Standardized cervical balance regulation and acupuncture for the treatment of cervical vertigo,easy for clinical application and promotion.Methods: Sixty four cases of clinical cases that met the criteria for inclusion in this study were selected and divided in the test group and the control group by randomized controlled method.The clinical efficacy was observed.The test group was treated with acupuncture and cervical balance regulation treatment,and the control group was treated with acupuncture.The acupuncture method was the same as the acupuncture method in the test group.Check the peak systolic velocity(PSV)of the basilar artery(BA)and bilateral vertebral artery(VA),determine the positive rate of neck rotation test,and record cervical vertigo symptoms and functional assessment scales(ESCV)score before treatment,followed by clinical treatment.The treatment is once every other day,3 times for 1 course of treatment,and the clinical effects were counted after 2 courses.Finally,check the PSV of the BA and VA,determine the positive rate of neck rotation test,and record ESCV score after treatment,the data were subjected to statistical calculation and comparative analysis to evaluate the efficacy before and after treatment.Results:1.the test group efficacy:8 cases were cured,12 cases were markedly effective,9 cases were effective,2 cases were ineffective,the total effective rate was 93.55%;the control group: 4 case was cured,8 cases were markedly effective.8 cases were effective,6 cases were ineffective,and the total effective rate was 80.00%.The rank sum test(P<0.05)was statistically significant;2.ESCV score:The test group scored 12.60 ± 2.34 before treatment,25.34 ± 4.03 after treatment,the control group scored 12.05 ± 2.29 before treatment,22.98 ± 4.57 fter treatment,the ESCV score after treatment was significantly higher than that before treatment,and there was a significant difference between the two groups(P<0.05).3.Transcranial Doppler ultrasound :The PSV of BA and bilateral VA in the two groups of patients increased significantly,there was a statistically significant difference between the two groups after treatment(P<0.05).The positive rate of the neck-turning test in the test group was 83.87% before treatment and 9.68% after the treatment.The positive rate of the neck-turning test in the control group was 83.33% before treatment and 36.67% after the treatment.The positive rate of neck rotation test was significantly reduced after treatment,there was a statistically significant difference between the two groups after treatment(P<0.05).Conclusion:1.Orientation and positive acupuncture combined with acupuncture treatment and cervical balance regulation have therapeutic effects on cervical vertigo,but the therapeutic effect of the test group is significantly better than that of the control group.2.Transcranial Doppler ultrasound can be used as a diagnostic basis for cervical vertigo,determine the positive rate of neck rotation test by detecting the blood flow velocity of BA and VA,provide objective criteria for the diagnosis and evaluation of cervical Cervcal Vertigo.
