| Objective:To discuss the clinical related factors on cervical vertigo, and summarize the clinical experience of Pro.Fan Binhua on treating Cervical Vertigo, preparing for the standardization of diagnose and treatment on it.The main symptom of Cervical vertigo(VBA) is dizziness, which attacks often and influence patient’s work and living quality. The reason of the attack is complicated and needed deeply studied. Pro.Fan Binhua has been worked in this area for more than 40years and he mainly focus on the clinical study of spine, especially on cervical vertigo. He regards there is basic difference between<Su Wen> and<Lin Shu>.He regards the mechanical of cervical vertigo is based on "Deficiency" and he made "San-Bu Tuina Fa" as the first threat of it..The Experience and the unique method is the main content of our acceding.Methods:With the method of following teacher in the clinic,70 cervical vertigo cases were collected as observed group and 70 cases of normal volunteer were collected as controlled, group. Both groups were asked to have TCD and X-Ray examination. We recorded the symptoms and physical examination signs, analysis the symptom specialties, and discuss the relativity of individual indicators.Results:The summaries of experience includes two aspects:academic thoughts and experience.1. The mechanism is based on Deficiency. It is understood that the deficiency means Qi deficiency. It leads to less promoting function of blood, so the brain lack of blood nutrition and it causes dizziness.2.The main principle of treating cervical vertigo is "ZHENG-YIN related, and misplace being corrected. The manipulation should reach the muscle and ligament layer, and the key point should be average, gentle, deep and force. Correct the joint is focus on C2-3. On the other hand,3D-CTA find the abnormal blood vessel and "San-Bu-TuiNa"Method should be used. 3.Focusing on prevent reoccurrence. Pro Fan often teaches patients to correct their wrong posture and habit, and teaches experience methods.In clinical part,this subject find out following results:1. After Logistic Correlation-Regression statistic test, we found that cervical vertigo was closely related with the abnormal blood flow speed of cervical artery and basilar artery, tense in upper part of cervical and disorder of little joints. (p<0.05). TCD shows both cervical artery and (or) basilar artery abnormal blood flow speed. Cases who is younger and has shorter history of vertigo usually shows faster blood flow speed,while cases who is older and has longer history of vertigo usually shows slower blood flow speed.2.TCD find out:patients shows fast or slow blood flow of speed, either on left or right side. Young patients or short history patients show more fast blood flow while old patients or long history patients show more slow blood flow. 3.The attack of cervical vertigo is related with abnormal blood flow on both sides of VA. 4.The mechanism of cervical vertigo on young patients is different from old patients. Young patients mainly focus on "Qi-stagnation " while old patients mainly focus on "Qi-deficiency". It leads to different manipulation and different formula.Conclusion:1.Cervical vertigo is closely related with vertebra-basilar blood supply.2. It is closely related that whether the function of blood vessel is compensated. Old patients is low functioned in this aspect, so the blood supply is difficult to change.3.The clinical related factor of cervical vertigo is abnormal blood flow speed of cervical artery and basilar artery. The TCM mechanism of cervical vertigo is "Upper Deficiency" We choose "san bu TUINA" method to treat cervical vertigo.4.Treating cervical vertigo should based on "research the reason and correct the mislocation". X-Ray and 3D-CTA should be applied during the treating course. |
